胃癌第8p淋巴结转移的相关因素分析

目的探讨胃癌第8p淋巴结转移的相关因素。方法回顾性分析该科2011年1月—2012年12月间行胃癌根治术且有第8p淋巴结清扫的93例胃癌患者的临床病理资料。结果 93例胃癌患者中第8p淋巴结转移率明显低于第8总体（P〈0.001）及第8a（P=0.001）,其淋巴结转移度与第8及第8a之间差异无统计学意义（P＆gt；0.05）。第8p阳性淋巴结与肿瘤部位（P=0.004）、大小（P=0.013）、Bormann分型（P=0.013）、T分期（P=0.000）、N分期（P=0.000）和TNM分期（P=0.000）均相关。多因素分析结果显示仅肿瘤的T分期（P=0.000）和N分期（P=0.005）与第8p淋巴结转移显著相关。结论第8p组淋巴结转移与肿瘤的T分期和N分期呈正相关。

Related factors study of No. 8 plymphnode metastasis in 93 patients with gastric cancer

Objective To explore the factors affecting lymphadenectomy behind common hepatic artery in gastric cancer by detection of metastatic rate and metastasis of lymphnodes of No. 8p subgroup. Methods The clinical data including complete information on No. 8p subgroup lymphnodes in 93 patients undergoing radical operation for gastric cancer between January 2011 to December 2012 were retro- spectively reviewed, and the relationship between the metastasis of No. 8p lymphnodes and clinicpathologic features were analyzed by lo- gistic regression analysis. Results The metastatic rate of No. 8p subgroup lymphnodes was 13.98% （13/93）. The metastatic rate of lymphnodes in No. 8p was significantly lower than that in No. 8（36.56% ,P 〈0. 001 ）and No8a（33.33% ,P =0. 001 ）. The metastasis of the resected lymphnodes in No. 8p was 28.95% （88/304）. The difference between No. 8 （27.46 % ）and No. 8a（ 28.95 % ） was not significant （ P 〉 0.05 ）. The metastasis of lymphnodes in No. 8p subgroup was correlated to location （ P = 0. 004 ） , size （ P = 0. 013 ） , Bonnann type （ P = 0.013 ）, T stage （ P = 0.000） , N stage （ P = 0. 000 ） and TNM stage （ P = 0. 000 ）. The logistic regression analysis showed that the metastasis of No. 8p lymphnodes was correlated only to depth of tumor invasion （P = 0. 000） and lymph node metastases （P = 0. 005 ）. There were no lymphadenectomy related complications, such as anastomotic leakage, lymphatiefistula and postoperative hemorrhage in this series. Conclusions The metastasis of No. 8p lymphnodes is positively correlated only to depth of tumor invasion and lymph node metastases.
