摘 要: | 目的运用电子喉镜对上气道形态、结构及表面特征的直接观察,Muller呼吸试验模拟上气道阻塞状态下咽腔塌陷情况推断睡眠时气道可能发生阻塞的部位,为睡眠呼吸暂停低通气综合症(OSAHS)病人的治疗提供参考。方法选取该院30例OSAHS病人和30例单纯鼾症对照者进行电子喉镜检查,分别在平静呼吸及Muller呼吸状态,观察其悬雍垂根部、扁桃体下极、会厌上缘平面塌陷程度,并对两组所观察结果进行比较。结果 OSAHS组较对照组有显著差异。OSAHS患者上气道狭窄表现在多个平面。不同程度OSAHS病人咽腔Muller氏试验下缩小范围有明显差异。结论电子喉镜对OSAHS患者上气道的阻塞平面及其变化观察具有重要的临床意义,是一种简便、有效的术前评估OSAHS病人上呼吸道情况的方法。

Abstract: | Objective To explore the possible obstructive site of the airway during sleep, so as to provide reference for the treatment of OSAHS patients. Methods We selected 30 OSAHS patients and 30 patients with simple snoring as controls to undergo electronic laryngoscopy. Roots of uvula, tonsil pole, epiglottis sink were observed and compared at quiet breathing and Muller respiratory status. Results There was a significant difference in OSAHS group from control group. Airway narrowing of OSAHS patients appeared in multiple planes. Narrowing degrees of pharynx of OSAHS patients were significantly different at Muller respiratory status. Conclusions Electronic laryngoscopy has clinical significance for observation of upper airway obstruction and change in OSAHS patients,which is a simple and effective preoperative assessment of OSAHS patients. |
