18F-FDG PET/CT显像联合病理对结直肠癌术后复发转移的评估

目的探讨结直肠癌组织中脉管癌栓、神经束侵犯、肠周淋巴结转移及肿瘤分化程度对肿瘤术后复发和转移的评估价值。方法收集PET/CT检查的76例结直肠癌术后患者的病理、临床及随访资料,回顾性分析病理标本中脉管癌栓、神经束侵犯、肠周淋巴结转移以及肿瘤分化程度与术后复发转移的关系。结果76例患者中57例出现复发转移(75%),19例无复发转移(25%)。复发转移患者与无复发转移患者在性别、分化程度、神经束侵犯等方面比较差异无统计学意义(P>0.05),但在脉管癌栓及肠周淋巴结转移方面比较差异有统计学意义(P<0.05)。结论脉管癌栓、肠周淋巴结转移可以作为结直肠癌术后复发转移的预测指标。

Evaluation of Postoperative Recurrence and Metastasis of Colorectal Cancer by 18F-FDG PET/CT Imaging Combined with Pathology

Objective To investigate the value of lymphovascular invasion,perineural invasion,local lymphatic metastasis and tumor differentiation in the recurrence and metastasis of postoperative colorectal cancer.Methods Pathological,clinical and follow-up information of 76 colorectal patients after surgery were collected.The relationship between lymphovascular invasion,perineural invasion,local lymphatic metastasis,tumor differentiation and prognosis were retrospectively studied.Results Among the 76 patients,57 cases(75%)had recurrence and metastasis and 19 cases(25%)had no recurrence and metastasis.Analysis indicated that there was no statistical difference in sex,differentiation,perineural invasion between the 2 groups(P>0.05),lymphovascular invasion and local lymphatic metastasis were significantly associated with recurrence and metastasis(P<0.05).Conclusion Lymphovascular invasion and local lymphatic metastasis can be used as one of the indicators to predict adverse postoperative colorectal cancer outcomes.
