人工股骨头置换术治疗高龄不稳定性股骨粗隆间骨折的护理

目的:探讨高龄非稳定性股骨粗隆间骨折患者行人工股骨头置换术的临床护理方法。方法:我院2007年1月～2009年12月对收治的16例年龄在75岁以上的股骨粗隆间非稳定性骨折患者施行人工股骨头置换术,术前进行心理护理,饮食指导,皮牵引护理,内科疾病护理;术后加强病情观察和并发症护理,早期指导功能锻炼。结果:16例患者术后4～8周能扶双拐行走,1年后能弃拐行走,随访6～28个月,髋关节功能恢复良好,无明显并发症发生,Harris评分优良率达87.5%。结论:对高龄非稳定性股骨粗隆间骨折行人工股骨头置换术患者加强卧床期护理,早期进行功能锻炼,能够有效地减少并发症,缩短卧床时间,促进患者早日康复,从而延长患者的生命,提高患者的生活质量。

Nursing of artificial femoral head replacement therapy instable intertrochanter fracture of femur with aged patients

Objective：To explore the clinical nursing of artificial femoral head replacement therapy instable intertrochanter fracture of femur with aged patients.Methods：16 patients of aged and instable intertrochanter fracture of femur treated by artificial femoral head replacement.Approach before operation,psychological nursing,diet guidance,skin traction nursing,internal diseases nursing will be conducted.After operation,observation of patient′s mental condition,nursing concentrated on complications,and earlier functional training will be given.Result：16 patients are able to walk with a pair of crutches four to eight weeks after surgery,they can walk without crutches one year later.All the patients obtained satisfactory joint function after 6 to 28 months follow-up.There were no complications,excellence rate of Harris evaluation was 87.5%.Conclusion：Strengthening clinic nursing of patients of aged and instable intertrochanter fracture of femur treated by artificial femoral head replacement and conducting earlier functional training can speed up the patient′s recovery after operation,shorten time in bed and reduce complication,which was significantly important to improve patients′ life quality and to prolong their life.
