聚羟基丁酸-羟基戊酸共聚酯膜与聚羟基丁酸-羟基戊酸共聚酯-溶胶-凝胶生物活性玻璃复合材料修复骨缺损的研究

摘    要:目的 探讨聚羟基丁酸-羟基戊酸共聚酯(PHBV)膜与PHBV-溶胶-凝胶生物活性玻璃(PHBV-SGBG)多孔复合材料促进骨再生的能力.方法 制备犬胫骨骨缺损模型,实验分4组:PHBV膜+PHBV-SGBG组;PHBV膜组;PHBV-SGBG组;空白对照组.术后2、4、8、12周取材,扫描电镜下观察各组骨缺损区的骨生长情况,并进行新骨的Ca、P元素微区定量测定.结果 所有植入材料组(PHBV膜+PHBV-SGBG组、PHBV膜组、PHBV-SGBG组)均可不同程度地促进新骨的形成.术后12周,PHBV膜+PHBV-SGBG组骨缺损区的修复重建基本完成,新生骨的Ca/P比值(1.625±0.037)与周围正常骨的Ca/P比值(1.625±0.063)差异无统计学意义(P>0.05);空白对照组的新生骨质疏松有洞,显示有较高的有机组织能谱;PHBV-SGBG组的骨修复略优于PHBV膜组,但两组间的差异无统计学意义(P>0.05).结论 PHBV膜具有引导骨再生的能力,PHBV-SGBG复合材料具有很好的骨传导性和骨形成能力,联合应用PHBV膜和PHBV-SGBG复合材料具有促进骨缺损修复的协同作用.

Abstract:Objective To evaluate the efficiency of poly hydroxybutyrate-hydroxyvalerate (PHBV) membrane and PH BV - sol - gel bioactive glass (PHBV - SGBG) composite for promoting bone regeneration.methods Canine tibial defect models were established and divided into four groups: PHBV membrane +PHBV-SGBG group, PHBV membrane group, PHBV-SGBG group and control group. Specimens were respectively collected from each group at 2, 4, 8 and 12 weeks after operation for bone growth assessment over the bone defect region by scanning electron microscopy. Quantitative determination of Ca and P over micro-areas of newly formed bone was also performed. Results New-bone formation in varying degrees was noted in all the treatment groups (PHBV membrane + PHBV-SGBG group, PHBV membrane group and PHBV-SGBG group). Repair and reconstruction over bone defect area was basically completed 12 weeks after operation in PHBV membrane + PHBV-SGBG group, with no statistical significance between Ca/P ratios of newly formed bone (1.625±0.037) and surrounding normal bone (1.625±0.063) (P>0.05). Newly formed bone in the control group appeared porous, indicating a relatively high energy spectrum of organic tissues.The discrepancy between PHBV-SGBG group and PHBV membrane group had no statistical significance although PHBV-SGBG group seemed to have better bone repair (P>0.05). Conclusion PHBV membrane induces bone regeneration, while PHBV-SGBG composite results in good bone conductivity and formation.Combined use of PHBV membrane and PHBV-SGBG composite shows a synergistic effect of repair of bone defects.
