梅尼埃病耳蜗电图特征分析

摘    要:目的 评价梅尼埃病患者耳蜗电图特征及其对梅尼埃病的诊断价值.方法 研究对象为52例梅尼埃病患者,其中男29例,女23例;年龄34~71岁,平均(51.2±10.6)岁.患者根据其治疗前纯音听力水平被分成临床早期组和临床中晚期组,所有患者其患耳和健耳均接受耳蜗电图检查,以-SP/AP≥40%判定其耳蜗电图异常,分析其耳蜗电图特征及其在梅尼埃病的诊断价值.结果 因2例系患有双侧梅尼埃病,故共计患耳54耳,健耳50耳.50只健耳中,未能引出耳蜗电图者6耳(12.0%),但在可引出耳蜗电图之健耳的耳蜗电图异常率为0.54只患耳中,未引出耳蜗电图者16耳,其未引出率(29.6%)明显高于健耳耳蜗电图未引出率(12.0%),P< 0.05;耳蜗电图正常者18耳(33.3%);而耳蜗电图异常者20耳,异常率为37.0%,其中临床早期组患者的耳蜗电图异常率(9/19,47.4%)高于临床中晚期组患者(11/35,31.4%),但差异无统计学意义;而临床早期组患者的耳蜗电图未引出率(1/19,5.3%)明显低于临床中晚期组患者(15/35,42.9%),P<0.05.结论 耳蜗电图可作为诊断梅尼埃病尤其是早期患者的辅助检查手段.

Abstract:Objective To investigate the characteristics of eleetrocochleography (ECochG) in patients with Meniere's disease (MD) and to evaluate the value of ECochG for the diagnosis of MD.Methods A total of 52 patients with MD,29 males and 23 females,aged from 34 to 71 years with an average of 51.2 years,were included in this study.According to the levels of hearing loss in the affected ears on pure-tone audiometry before treatment,patients were divided into early hearing loss group and advanced hearing loss group.All patients underwent ECochG examinations for the affected ears and healthy ears.When-SP/AP ≥ 40%,the ECochG was determined as abnormal.Results Among 52 patients,2 patients had bilateral MD and so there were 54 affected ears and 50 healthy ears.Of healthy ears,6(12.0%) ears did not show in ECochG and other ears showed normal ECochG,no ear showed abnormal ECochG.Among the affected ears,ECochG was not detected in 16 (29.6%) ears,which was higher than the rate (12.0%) of healthy ears (P < 0.05);16(33.3%) ears showed normal ECochG;20 (37.0%) showed abnormal ECochG,of them,9(47.4%) occurred in 19 ears with early MD and 11(31.4%) in 35 ears with advanced MD,with no significant difference (P > 0.05);and ECochG was not detected in 1(5.3%) of 19 ears with early MD,which was significantly less than 15(42.9%) of 35 ears with advanced MD (P < 0.05).Conclusion ECochG is an effective diagnostic test for MD especially in early MD.
