老年人糖尿病周围神经病变危险因素的探讨

目的 研究老年糖尿病人群在血糖控制达标率高的情况下糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的危险因素.方法 随机选取符合标准的≥60岁社区老年糖尿病214例为研究对象,按照是否患有DPN,DPN组和非DPN组,观察DPN与各观察指标的关系.再按照年龄段分为3组:60～岁组、70～岁组及80～岁组,比较不同年龄段的DPN患病率.结果 DPN组与非DPN组间的单因素比较,年龄和HbA1c组间差异有统计学意义(P＜0.05).多因素Logistic回归分析结果,只有年龄进入回归方程(P＜0.01),其OR值为1.187,95％CI为1.105～1.275.60～岁组、70～岁组及80～岁组DPN患病率的差异具有统计学意义(P＜0.01),两两比较(α'=0.017),各组间差异均具有统计学意义(P＜ 0.017).结论 年龄可能是老年人群糖尿病周围神经病变的独立危险因素.

Study on the risk factors of diabetic peripheral neuropathy in old people with diabetes mellitus

Objective To explore the possible risk factors for peripheral neuropathy in elder patients with well-controlled blood glucose level.Methods 214 patients were randomly selected among elder residents of a community.All patients met the inclusion criteria and aged over 60 years ole.They were divided into the DPN group and non-DPN group based on whether they had DPN or not.They were further stratified according to ages to aged 60,70 and 80 yeard and older groups.The prevalence rate was compared between different age groups.Results Single-factor analysis showed that there was significant difference in age and HbA1C level in patients with and without DPN(P ＜ 0.05).Multi-factor regression analysis reveled that age was the risk factor for DPN(P ＜ 0.01) with the OR was 1.187,95％CI ranged from 1.105 to 1.275.The statistical significant difference was found among the three groups (P ＜ 0.01).There was significant difference in the prevalence rate of DPN between the 3 age groups (α'=0.017).Conclusion Age is an independent risk factor for diabetic peripheral neuropathy in old people with diabetes mellitus.
