颅脑创伤后神经行为变化及其与局部脑血流量变化的关系

摘    要:目的探讨颅脑创伤后神经行为变化的原因。方法应用气体冲击致伤装置,制成大鼠中等度颅脑创伤后,观察伤后不同时间的神经行为变化,同时以氢清除法测定伤区皮层、伤侧海马、对侧皮层及对侧海马的脑血流量。结果大鼠伤后1天,其受伤对侧肢体就有明显的神经行为改变;而伤区皮层及伤区海马的局部脑血流量则在伤后30分钟至伤后6小时呈明显下降,伤后1小时下降最为明显。结论颅脑创伤后神经行为变化的原因是多方面的,伤区皮层及伤侧海马局部脑血流量下降可能是其原因之一。此结果提示,迅速纠正伤后的缺血是治疗颅脑创伤的重要方面。

Abstract:Objective To discuss the causes of neurobehavioral changes following traumatic brain injury. Methods The moderate brain injury of rats were produced by gas percussion. The neurobehavioral changes after traumatic brain injury were observed. The reginal cerebral blood flow (rCBF) of injured cortex, ipsilateral hippocampus, contralateral cortex and hippocampus was measured by hydrogen clearance method. Results It was observed that the distinct neurobehavioral changes of contralateral limbs at 1 day after trauma, and the rCBF of injured cortex and ipsilateral hippocampus was significantly decreased during 30 minutes to 6 hours after trauma, it decreased to the lowest at 1 hour after trauma. Conclusion There are many causes of neurobehavioral changes after traumatic brain injury, The decreased rCBF of injuried cortex and ipsilateral hippocompus may be one of these. The results indicated that it be important to treat TBI that prompt correcting focal oligemia posttrama.
