全程优质护理模式对初产妇分娩质量、泌乳功能、满意度及舒适度的影响

目的 探讨全程优质护理模式对初产妇分娩质量、泌乳功能、满意度及舒适度的影响.方法 选择在浙江省湖州市妇幼保健院及湖州师范学院附属第一医院住院的初产妇60例,按随机数字表法分为对照组和观察组.对照组采用常规初产妇专业护理措施;观察组实施全程优质护理模式.观察比较两组产妇剖宫产率、产后泌乳始动时间、泌乳量评分及产妇满意度等情况.结果 观察组产妇剖宫产率（10.00％）低于对照组产妇（33.33％）,差异有统计学意义（P＜0.05）.观察组初产妇产后泌乳始动时间明显早于对照组,差异有高度统计学意义（P＜0.01）.观察组初产妇产后24、48 h泌乳量评分显著高于对照组,差异有统计学意义（P＜0.05或P＜0.01）.观察组初产妇对分娩的主观满意度为80.00％,高于对照组初产妇对分娩的主观满意度（53.33％）,差异有统计学意义（P＜0.05）.观察组初产妇的围术期重度不适率（40.00％）低于对照组（66.67％）,差异有统计学意义（P＜0.05）.结论 全程优质护理模式能够降低初产妇剖宫产率,改善产妇产后的泌乳功能,并能够明显改善初产妇对分娩的满意度及舒适度.

Effect of the entire process model of quality nursing care intervention on quality of maternal delivery,lactation, satisfaction and comfort in primipara

Objective To discuss the effect of the entire process model of quality nursing care intervention on quality of maternal delivery,lactation,satisfaction and comfort in primipara.Methods 60 primipara in Huzhou Maternal and Child Health-Care Center and the First Affiliated Hospital of Huzhou Teachers College were randomly divided into control group and observation group.The control group was given conventional primipara nursing measures.The observation group was given the entire process model of quality nursing care.The cesarean section rate,postpartum initial time of lactation,lactation yieldm score,maternal satisfaction were compared between the two groups.Results Maternal cesarean section rate of the observation group was 10.00％,lower than 33.33％ in the control group,the differences were statisfaction significant （P ＜ 0.05）.The postpartum initial time of lactation of the observation group was earlier than that of control group,the difference was statistically significant （P ＜ 0.01）.Lactation yieldm scores of primipara 24,48 h after delivery in the observation group were better than in the control group,the differences were statisfaction significant （P ＜ 0.05 or P ＜ 0.01）.Primipara subjective satisfaction in childbirth of the observation group was 80.00％,higher than 53.33％ in the control group,the difference was statistically significant （P ＜ 0.05）.Primipara severe discomfort rate of perioperative period was 40.00％ in observation group,less than 66.67％ in the control group,the difference was statistically significant （P ＜ 0.05）.Conclusion The entire process model of quality nursing care can reduce the trend of primiparaous cesarean section rates,improve maternal postpartum lactation,and obviously improve the satisfaction and comfortable degree of delivery.
