康复护理干预对肱骨外科颈骨折患者术后治疗依从性、肩关节功能恢复和护理满意度的影响

目的 探讨康复护理干预对肱骨外科颈骨折患者术后治疗依从性、肩关节功能恢复和护理满意度的影响.方法 选择2009年1月～2013年4月浙江省上虞市人民医院骨科住院接受切开复位内固定治疗的肱骨外科颈骨折的患者72例,随机分为常规护理组和康复护理组,每组各36例.两组患者均行肱骨外科颈切开复位内固定术.常规护理组予以围术期常规护理,康复护理组予以早期康复护理干预.两组患者术后随访12周,观察并比较两组患者治疗依从性、患侧肩关节功能和护理满意度情况.结果 随访12周后,康复护理组的治疗总依从率为91.67％,明显高于对照组（72.22％） （x2=4.60,P＜0.05）;康复护理组患者术后肩关节功能恢复优良率为94.44％,明显高于常规护理组（77.78％）（x2=4.18,P＜0.05）;且康复护理组患者的服务态度、技术水平、健康宣教、关爱患者、心理支持等五项的护理满意度（97.22％、94.44％、97.22％、94.44％、91.67％）均明显高于常规护理组（75.00％、77.78％、72.22％、75.00％、69.44％）（P＜ 0.05或P＜0.01）.结论 康复护理干预有利于提高肱骨外科颈骨折术后治疗依从性,促进患者术后肩关节功能恢复,提高手术的疗效,能最大程度上减轻患者肩关节功能恢复障碍,并能提高术后护理满意度,建立更紧密的护患关系.

Influence of recovery nursing intervention on treatment compliance,postoperative shoulder joint recovery and nursing satisfaction of fracture of humeral surgical neck

Objective To discuss influence of recovery nursing intervention on treatment compliance,postoperative shoulder joint recovery and nursing satisfaction of fracture of humeral surgical neck.Methods 72 cases of patients with fracture of humeral surgical neck,who were treated with open reduction and internal fixation （ORIF） in Department of Orthopedics of Shangyu People＇s Hospital in Zhejiang Province,during the period from January 2009 to April 2013,were divided into routine nursing group （n=36） and recovery nursing group （n=36） at random.The patients in two groups were treated with ORIF of fracture of humeral neck.The patients in routine nursing group were given routine nursing in perioperative period,while the patients in recovery nursing group were given early recovery nursing intervention.The patients in two groups were followed up for 12 weeks after the operation,treatment compliance,affected shoulder joint function and nursing satisfaction of patients in two groups were observed and compared.Results After 12 weeks＇ following-up,the total compliance efficiency of patients in recovery nursing group after the operation was 91.67%,which was much higher than that in routine nursing group （72.22%） （x2--4.60,P 〈 0.05）; the recovery good and excellent rate of shoulder joint function of patients in recovery nursing group after the operation was 94.44%,which was much higher than that in routine nursing group （77.78%） （x2=4.18,P 〈 0.05）.Further,the nursing satisfaction of service attitude,technical level,health education,patients caring and mental support in recovery nursing group （97.22%,94.44%,97.22%,94.44%,91.67%）,was much higher than that in routine nursing group （75.00%,77.78%,72.22%,75.00%,69.44%） （P 〈 0.05 or P 〈 0.01）.Conclusion Recovery nursing intervention can improve obviously treatment compliance of humeral surgical neck,is favorable for the recovery of shoulder joint function of fracture and improve the curative effect of operation,which can reduce the recovery barrier of shoulder joint of patients,enhance the postoperative nursing satisfaction and build the close relationship between nurses and patients.
