2 种 预防宫腔镜手术后宫腔再粘连方法的对比分析

目的对比分析Bakri球囊及Interceed防粘连膜用于中-重度宫腔粘连宫腔镜手术后预防再粘连的治疗效果。方法选择2010年6月～2013年6月佛山市禅城区中心医院妇科中-重度宫腔粘连患者105例为研究对象，将其分为Bakri球囊组（42例）、Interceed防粘连膜组（43例）及对照组（20例）。Bakri球囊组使用Bakri球囊管＋戊酸雌二醇（E_2V）＋宫内节育器（IUD）；Interceed组使用Interceed防粘连膜＋E_2V＋IUD；对照组使用E_2V＋IUD。术后观察宫腔粘连、月经及子宫内膜恢复情况。结果Bakri球囊组和Interceed防粘连膜组的总有效率都达到100．0％，均高于对照组（80．0％），差异有统计学意义（P〈0．05），其中，Interceed防粘连膜组的显著率达到37．2％，高于Bakri球囊组（26．2％），差异有统计学意义（P〈0．05）。Bakri球囊组月经恢复正常者32例（76．2％），Interceed防粘连膜组月经恢复正常者34例（79．1％），对照组月经恢复正常者11例（55．0％），差异有统计学意义（P〈0．05）。结论Interceed防粘连膜更适用于宫腔镜手术后宫腔粘连的预防，且无影响月经恢复的副作用。

The analysis of two kinds of methods in prevention of intrauterine adhe- sions after hysteroscopic surgery

Objective To compare and analyze the effect of Bakri balloon and Interceed anti adhesion membranes on the prevention of intrauterine adhesions after hysteroscope operation among patients with moderate to severe intrauterine adhesions. Methods 105 cases of moderate to severe intrauterine adhesions from June 2010 to June 2013 in Chaneheng District Central Hospital of Foshan City were selected as study objects, and they were randomly divided into three groups： control group （20 cases）, Bakri balloon group （42 cases） and Intereeed anti adhesion membranes group （43 cases）. Patients of control group were treated with E2V＋IUD, Bakri balloon group were treated with Bakri balloon＋E2V＋ IUD and the Interceed anti adhesion membranes group were treated with Interceed anti adhesion membranes＋E2V＋IUD. And the intrauterine adhesions, menstruation and endometrial recovery were tested. Results The effective rate of Bakri balloon group and Interceed anti adhesion membranes group reached 100.0%, which was remarkably higher than that of the control group （80.0%）, the difference was statistically significant （P 〈 0.05）. In addition, the effectual rate of Inter- ceed anti adhesion membranes group （37.2%） was higher than that of Bakri balloon group （26.2%）, the difference was statistically significant （P 〈 0.05）. The number of amenorrhea recovered case in control group, Bakri balloon group and Interceed anti adhesion membranes group was 55.0%, 76.2% and 79.1% respectively, the difference was statistically significant （P 〈 0.05）. Conclusion The Interceed anti adhesion membranes is a better preventive against intrauterine adhesions after hysteroscopic operation, and has no side effect on the repair of uterine endometrium.
