安非他酮缓释片治疗抑郁症患者的效果及对血清 P 物质和神经肽 Y 水平的影响

目的 探讨安非他酮缓释片对抑郁症患者血清P物质（SP）和神经肽Y（NPY）水平的影响及疗效观察.方法 将70例抑郁症患者随机分为实验组与对照组,分别予以安非他酮缓释片和氟西汀治疗6周,观察两组治疗前后血清SP和NPY水平变化,并比较其疗效及不良反应.结果 治疗6周后,两组血清SP水平[（40.12±12.73）、（48.43±12.17） ng/L]均较治疗前[（55.17±14.76）、（54.97±15.01）ng/L]明显下降,NPY水平[（138.4923.17）、（119.53±22.86）ng/L]均较治疗前[（104.64±21.37）、（103.26±20.56）ng/L]明显上升（P＜ 0.05或P＜0.01）,且实验组下降或上升值明显大于对照组（P＜0.05）;同时实验组患者的临床总有效率（94.3％）明显优于对照组（77.1％）（x2=4.20,P＜0.05）,不良反应发生率（14.3％）明显低于对照组（37.1％）,差异有统计学意义（x2=4.79,P＜0.05）.结论 安非他酮缓释片治疗抑郁症效果确切,安全性较好,其作用机制与其能降低血清SP水平及升高NPY水平密切相关.

Influence and curative effect observation of Zyban Sustained Release Tablet on serum substance P and neuropeptide Y levels of depression

Objective To discuss influence and curative effect observation of Zyban Sustained Release Tablet on serum substance P （SP） and neuropeptide Y （NPY） levels of depression.Methods 70 cases of depression patients were selected and divided into experimental group and control group at random.The patients in experimental group were given Zyban Sustained Release Tablet,while the patients in control group were given Fluoxetine.The period of treatment in was 6 weeks.The changes of serum SP and NPY levels of patients in two groups before and after medical treatment were observed,and the curative effect and untoward effect was compared.Results After 6 weeks＇ medical treatment,serum SP level of patients in two groups [（40.12±12.73）,（48.43±12.17） ng/L] were obviously declined than before [（55.17±14.76）,（54.97±15.01） ng/L],while the NPY level [（138.49±23.17）,（119.53±22.86） ng/L] were obviously rose than before [（104.64±21.37）,（103.26±20.56） ng/L] （P ＜ 0.05 or P ＜ 0.01）,and the declining or rising rate in experimental group were much higher than those in control group （P ＜ 0.05）.The total clinical efficiency in experimental group （94.3％） was much higher than that in control group （77.1％） （x2=4.20,P ＜ 0.05）.The occurrence rate of untoward effect in experimental group （14.3％） was much lower than that in control group （37.1％） （x2=4.79,P ＜ 0.05）.Conclusion Zyban Sustained Release Tablet has reliable curative effect on depression with high security,whose mechanism of action has close effect on reducing serum SP level and rising NPY level.
