医用透明质酸钠预防大孕龄人工流产术后宫腔粘连的效果观察

目的探讨医用透明质酸钠预防大孕龄人工流产术后宫腔粘连的临床效果。方法选取2012年1月～2013年12月在浙江省台州市中医院院就诊的超12周孕龄的人工流产患者为研究对象，将200例患者随机分为观察组和对照组，每组各100例，对照组患者采用传统的负压吸引术进行治疗，观察组患者在对照组的基础上术后给予医用透明质酸钠治疗，比较两组患者的术后腹痛持续时间、阴道流血量、感染情况、术后月经情况、宫腔粘连情况。结果观察组患者的术后腹痛持续时间短于对照组，阴道流血量少于对照组，感染率低于对照组，差异有统计学意义（P〈0．05）。观察组患者的月经复潮时间为（28．7±5．5）d，明显短于对照组的（35．7±8．4）d，宫腔积血率、闭经率及周期性腹痛发生率均低于对照组，差异有统计学意义（P〈0．05）。观察组患者宫腔粘连的发生率为4．0％，明显低于对照组的20．0％，差异有统计学意义（P〈0．05）。结论医用透明质酸钠凝胶可有效预防大孕龄人工流产术后宫腔粘连，可降低并发症的发生率，效果显著。

Effect observation of medical Sodium Hyaluronate to prevent large post- operative gestational abortion caused by intrauterine adhesions

Objective To explore the clinical effect of medical Sodium Hyaluronate to prevent large postoperative ges- tational abortion caused by intrauterine adhesions. Methods The patients with gestational age of abortion treated in Taizhou Hospital of Traditional Chinese Medicine from January 2012 to December 2013 were selected as the research object, the 200 patients were randomly divided into observation group and control group, 100 cases in each group. The control group was treated by traditional negative pressure aspiration for treatment, the observation group was given medical Sodium Hyaluronate postoperatively on the basis of the control group. The duration of postoperative abdominal pain, vaginal bleeding, infection situation and situation of postoperative period, caused by intrauterine adhesions and after preoperative microcirculation in the two groups were compared. Results The postoperative abdominal pain dura- tion in observation group was shorter than that of the control group, the vaginal bleeding amount was less than that of the control group, the infection rate was lower than that of the control group, the differences were statistically signifi- cant （P 〈 0.05）. The menstruation after tide time in observation group was （28.7＋5.5） d, significantly shorter than the （35.7_＋8.4） d in control group, the incidence of uterine cavity hemorrhage rate, rate of amenorrhea and periodic abdomi- nal pain were lower than those of the control group, the differences were statistically significant （P 〈 0.05）. The inci- dence rate of observation group patients caused by intrauterine adhesions was 4.0%, significantly lower than the control group （20.0%）, the difference was statistically significant （P 〈 0.05）. Conclusion The medical Sodium Hyaluronate Gel can effectively prevent large postoperative uterine cavity adhesion, gestational abortion can reduce the incidence rate of complications, the effect is remarkable.
