ICU 综合征发生的基础疾病危险因素分析

目的研究ICU综合征发生的基础疾病危险因素。方法将首都医科大学附属北京世纪坛医院2008年1月一2013年6月ICU收治的1560例患者，根据有无发生ICU综合征分为病例组（298例）和对照组（1262例），并对危险因素进行分析。结果1560例患者ICU综合征发生率为19．1％（298例）；病例组的年龄、住院时间、人住ICU时间明显高于对照组；具有脑血管疾病、药物中毒等基础疾病的患者发生率高；内科疾病患者在人住ICU48h内发生率明显高于同期的外科患者，而48h后两组差异无统计学意义（P〉0．05）；病例组患者术后并发症的发生率高于对照组，其中各种肠瘘比较，差异有统计学意义（P〈0．05）；住院病死率和手术所用时间明显高于对照组，差异有统计学意义（P〈0．05）。结论ICU患者发生ICU综合征的危险因素包括年龄、脑血管病变和药物中毒等基础病、术后并发症等。

Analysis of underlying diseases risk factors of ICU syndrome

Objective To analyze underlying diseases risk factors of ICU syndrome. Methods From January 2008 to June 2013, 1560 patients treated in ICU of Beijing Shijitan Hospital Affiliated to Capital Medical University were se- lected, and divided into study group （298 cases） and control group （1262 cases） depended on whether they suffered ICU syndrome. The risk factors of it were studied. Results 298 of 1560 patients （19.1%） were suffered ICU syndrome. The age, hospital staying time and ICU staying time of the study group were higher （longer） than those of the control group, the differences were statistically significant （P 〈 0.05）. The patients with cerebrovascular disease and drug poisoning were prone to suffer ICU syndrome. More patients from the department of internal were prone to suffer ICU syndrome in the first 48 h than those from the department of surgical, however, there was no difference after 48 h （P 〉 0.05）. The patients of studygroup were more complications than those of control group, especially more intestinal fistula, higher in-hospital mortality and longer operating time than those of controlling group, the differences were statistically signifi- cant （P 〈 0.05）. Conclusion The ICU syndrome risk factors of ICU patients are age, cerevasular disease, complication and drug poisoning.
