小儿外科手术中丙泊酚联合芬太尼麻醉对患儿呼吸循环的影响

目的探讨小儿外科手术中丙泊酚联合芬太尼麻醉对患儿呼吸循环的影响。方法选择攀枝花市中心医院2010年10月～2013年10月收治的接受外科手术的患儿80例进行研究，随机分为两组，观察组采用丙泊酚联合芬太尼麻醉，对照组采用丙泊酚联合异氟烷麻醉。观察记录两组患者的心率（HR）和收缩压（SBP）、舒张压（DBP）以及呼吸频率（RR）、血氧饱和度（SpO2）变化情况，并进行比较。结果观察组的HR和SBP、DBP以及RR、SpO2的平稳程度明显高于对照组（均P〈0．05）。观察组患儿术后呼吸恢复时间为（4．01±2．21）min，拔管时间为（6．12±3．74）min，离开恢复室时间为（13．26±5．11）min，均显著短于对照组，差异均有统计学意义（均P〈0．05）。结论在小儿外科手术中采用注丙泊酚联合芬太尼的方法对患儿呼吸循环的影响较小，可以达到较好的麻醉效果。

Effect of Propofol combined with Fentanyl anesthesia on children with respiratory cycle operation in pediatric surgery

Objective To analyze the effects of Propofol in pediatric surgery operation combined with Fentanyl on respiratory and circulatory effects in children. Methods 80 cases of patients with surgical operation in Panzhihua Central Hbspital from October 2011 to October 2013 were selected and randomly divided into two groups. The observation group was treated with Fentanyl and Propofol anesthesia, the control group was treated with Propofol combined with Isoflurane anesthesia. The changes of heart rate （HR） and systolic blood pressure （SBP）, diastolic blood pressure （DBP）, and respiratory rate （RR）, oxygen saturation （SpO2） of the two groups were tested and compared. Results The smooth degree of HR and SBP, DBP, RR, and SPO2 of the observation group were all higher than those of control group significantly （all P 〈 0.05）. The postoperative respiratory recovery time of the observation group was （4.01±2.21） min, the extubation time was （6.12±3.74） min, the leaving the recovery room time was （13.26±5.11） min, which were all significantly shorter than those of the control group （all P 〈 0.05）. Conclusion The effect on respiratory cycle is less in the pediatric surgical operation by giving the children injection of Propofol combined with Fentanyl, which can achieve good effects of anesthesia.
