有机磷农药中毒患者一体化急救护理管理探讨

目的探讨一体化急救模式下护理管理在有机磷农药中毒患者救治中所发挥的作用。方法将198例有机磷农药中毒患者按时间先后分为两组，2012年5月-2013年12月浙江省余姚市人民医院救治的96例有机磷农药中毒患者设为实验组，2010年9月-2012年4月浙江省余姚市人民医院救治的102例有机磷农药中毒患者设为对照组，实验组实施与一体化急救模式相适应的护理管理措施，包括调整急救中心的布局与设施、完善应急预案、规范抢救程序、注重抢救的时效性、优化人力资源、规范护理病历书写、定期组织学习法律知识等．对照组常规护理，未实施与一体化急救模式相适应的护理管理措施，通过回顾分析比较两组患者的救治效果和满意度。结果与对照组相比，实验院前反应时间明显缩短，差异有高度统计学意义[（3．42±1．14）min比（5．68±1．34）min，P〈0．01）]，中毒症状消失时间、胆碱酯酶恢复到60％时间、住院时间均缩短，差异有统计学意义（P〈0．05），住院费用减少，差异有统计学意义[（14889±3366）元比（20448±4181）元，P〈0．05），救治成功率和救治满意度均提高，差异有统计学意义（96．9％比91．2％，97．9％比93．1％，P〈0．05）。结论利用急救一体化模式的优势，不断完善急救护理管理，提供及时、有效、科学的现代急救护理，提高了有机磷农药中毒患者的救治水平，提升了医院的综合急救素质。

Explorations about organophosphorus pesticide poisoning patients with integrated emergency nursing management

Objective To explore the integration of emergency nursing management mode in treating patients with organophosphorus pesticide poisoning. Methods 96 patients with organophosphorus pesticide poisoning from May 2012 to December 2013 were setted in the experiment group, 102 patients with organophosphorus pesticide poisoning from September 2010 to April 2012 were setted in the control group. The experiment group was adapted to the implementa- tion and integration of emergency rescue mode of nursing management measures, including adjustment of the emergen- cy center layout and facilities of the emergency center, improve the emergency response plans, salvage procedures, pay attention to the timely rescue, optimization of human resources, standard care medical record writing, organize regular learning legal knowledge. The control group was given the conventional nursing, without implement care management and integrated emergency mode to adapt, through retrospective analysis comparing two groups of patients the treatment effect and satisfaction. Results Compared with control group, in experiment group, the reaction time was significantly shorten [（3.42±1.14） min vs （5.68±1.34） min], the difference was significant （P 〈 0.01）. Poisoning symptoms disappear time, time of cholinesterase restored to 60%, and the hospitalization time were shorten, the differences were significant （P 〈 0.05）. Hospitalization cost reduced [（14 889±3366） yuan vs （20 448±4181） yuan], the difference was significant （P 〈 0.05）. Treatment success rate and treatment satisfaction both increased with significant differences （96.9% vs 91.2%, 97.9% vs 93.1%）. Conclusion Advantage of emergency integration mode, perfecting the emergency nursing management, provide timely, effective and scientific modem emergency nursing, can improve the treatment level of the patients with organophosphorus pesticide poisoning and comprehensive emergency qualities of the hoslaital.
