电视胸腔镜手术在胸外科应用的临床研究

目的 :探讨电视胸腔镜手术 (VATS)在胸外科领域的适应症和应用前景。方法 :回顾分析 1998年 7月～ 2 0 0 2年 12月 78例各类胸腔镜手术的临床资料。结果 :78例VATS手术中无 1例中转开胸 ,5例因胸膜顶粘连辅助小切口 ,3例中老年自发性气胸术后持续漏气 ,分别于第 7、8、13d拔除胸管 ,其它均在 4 8h内拔除胸管。 5例引流管口延期愈合。手术并发症发生率 10 .3% (8/ 78)。结论 :VATS在胸外科有广阔的发展空间 ,一次性耗材价格昂贵 ,限制其临床应用 ,腔镜下缝合技术或打结技术的应用可节省医疗费用

The use of video-assisted thoracoscopy in thoracic surgery

Objective To study the indications and techniques of video-assisted thoracoscopic surgery(VATS)in treatment of thoracical diseases.Methods 78 patients underwent traditional three trocars or assisted mini-incision VATS for spontaneous pneumothorax(40),chest trauma(12),solitary pulmonary nodules(9),pleural diseases(6),mediastinal tumor(4),lung cancer(4),broncho-pleural fistula(2) and esophageal myoma(1) from July 1998 to December 2002.Results The mean operative blood loss 70 ml(10 ml～600 ml).The mean postoperative chest drainage time was 2.4 days(1～13 d).The mean postoperative hospitalization time were 8.5 days(4～30 d).The overall incidence of postoperative complications was 10.3%(8/78).There was no recurrence of spontaneous pneumothorax and lung cancer.Conclusion Video-assisted thoracoscopic surgery is a safe and effective technique in selected patients with chest diseases.
