前房放液联合局部药物治疗白内障超声乳化人工晶状体植入术后早期高眼压

目的探讨前房放液联合药物治疗白内障超声乳化人工晶状体植入术后早期高眼压的有效性和安全性。方法东阳市人民医院于2007年9月—2010年10月间内行白内障超声乳化人工晶状体植入术1210例1310眼,术后早期发生高眼压27眼,其中无术中并发症发生17眼,随机分为药物观察组(药物组,8眼)及前房放液联合药物治疗组(放液组,9眼)。应用Goldman压平眼压计测量治疗前后的眼压,比较两组治疗后眼压控制的情况。结果两组治疗前眼压的差异无统计学意义(P>0.05)。治疗后3、6、24h,放液组的眼压显著低于药物组(P值均<0.05);治疗后48h,两组间眼压的差异无统计学意义(P>0.05)。治疗24h后,药物组眼压在正常范围内4眼(4/8),放液组眼压在正常范围内8眼(8/9)。结论前房放液联合药物治疗白内障超声乳化人工晶状体植入术后早期高眼压,具有降眼压幅度大、作用快速的优势,是一种有效和安全的治疗方法。

Anterior chamber paracentesis combined with topical medication for early intraocular hypertension after phacoemulsification cataract extraction and intraocular lens implantation

Objective To evaluate the efficacy and safety of anterior chamber paracentesis combined with topical medication for early intraocular hypertension after phacoemulsification and intraocular lens implantation in cataract patients.Methods Totally 1 210 patients(1 310 eyes) underwent phacoemulsification cataract extraction and intraocular lens implantation between September 2007 and October 2010.Intraocular hypertension occurred in 27 eyes after treatment.Of them,17 eyes without intraoperative complications were randomly divided into group A(general medication observation,8 eyes) and group B(anterior chamber paracentesis and topical medication,9 eyes).Intraocular pressure(IOP) was measured before and after treatment by Goldman applanation tonometer in both groups.Results The differences of IOP between the two groups before treatment and 48 hours after treatment were not statistically significant(P>0.05).But IOP in group B was significantly lower than that in group A 3,6,and 24 hours postoperatively(all P<0.05).IOP of 4 eyes in group A and 8 eyes in group B restored to the normal range 24 hours after treatment.Conclusion Anterior chamber paracentesis combined with topical medication can be considered as an effective and safe method to treat the early intraocular hypertension after phacoemulsification and intraocular lens implantation.
