彩色多普勒超声检查对滋养叶疾病化疗效果的评价

摘    要:目的:探讨在滋养叶疾病的治疗过程中痛灶区动脉血流阻力指数(RI)的变化及病灶大小对评判滋养叶疾病化疗效果及疾病转归的价值.方法:回顾性分析超声检查结果和临床资料完整的经病理证实的滋养叶疾病41例,对该患者进行彩色多普勒及脉冲多普勒超声检查,并记录病灶大小及子宫病灶区动脉血流阻力指数(RI),对化疗前后病灶大小及子宫病灶区动脉RI相关性进行统计学分析.结果:41例滋养叶疾病患者化疗前超声检查可见子宫增大,宫腔内见有混合回声,部分宫壁可见低回声区,血流丰富,RI:0.21~0.48;化疗取得疗效后异常血流逐渐减少,RI呈现逐渐上升的趋势,病灶范围缩小.结论:利用彩色多普勒超声检查对滋养叶疾病的诊断和化疗效果评价具有重要意义.

Abstract:Objective: explore the value of measuring the change of RI for artery blood flow in the focus area and the size of the focus area to the chemotherapy effect and disease transform identification during the treatment of trophoblastic diseases.Method: retrospectively analyzing with complete supersonic data the confirmed 41 cases of trophoblastic diseases through pathology analysis, including 38 cases first diagnosed in our hospital,3 cases diagnosed in other hospital but took chemotherapy in our hospital. Color Doppler ultrasonic examination and Impulse Doppler ultrasonic examination are conducted to the 41cases, then we record the size of the focus areas and the changes of RI for artery blood flow, to take statistic analysis to the correlation of these results before chemotherapy and after chemotherapy. Result: the hysteras increase of the 41cases, and there is mixed echo in the uterine cavity, and low echoing area exists at some part of the uterine wall, with rich blood flow. Rh 0.21- 0.48. After the effects of chemotherapy, abnormal blood diminishes, and the curve of RI takes on the increasing trend, with the diminishing of the focus area.Conclusion: it is significant for the diagnosis and chemotherapy monitoring of trophoblastic diseases to employ Color Doppler ultrasonic examination to measuring the change of RI for artery blood flow in the focus area and the size of the focus area of trophoblastic diseases before and after the chemotherapy, especially the focus area of trophoblastic diseases.
