彩色多普勒超声对急性阑尾炎的诊断价值

目的:结合急性阑尾炎的病理分型,分析声像图特征,以提高对其诊断准确率。方法:对127例急性阑尾炎病例,应用彩色多普勒技术及凸阵、线阵探头并结合临床资料综合分析,总结不同病理分型的声像图表现。结果:127例急性阑尾炎患者中,超声诊断为急性阑尾炎的113例,漏诊14例。声像图主要表现为阑尾自身和阑尾周围不同程度的改变。所有病例均经手术或随访证实。结论:彩色多普勒超声对急性阑尾炎的诊断有较高的准确率并可初步提示阑尾炎的分型,可作为协助临床对急性阑尾炎的诊断及选择治疗方案的首选影像诊断方法。

Value of color Doppler ultrasound in diagnosis of acute appendicitis

Objective： to analyze ultrasonographic characteristics, on the basis of pathological type of acute appendicitis, in order to improve diagnostic accuracy. Method： 127 cases of acute appendicitis were adpoted color doppler ultrasound, convex array and linear array probe combined with comprehensive analysis of clinical data, to summarize untrasonographic characteristics of different pathological types. Result： among 127 acute appendicitis patients, 113 cases were diagnozed ultrasonically as acute appendicitis, and 14 missed diagnosis. Among 127 cases, 39 were acute simple appendicitis, 67 were acute suppurative appendicitis, 13 were acute gangrenous ap- pendicitis inflammatory, and 8 were abscess of appendix package or appendicitis block. Ultrasonographic repre- sentations are mainly variation of appendicitis itself and surrounding area, each type of appendicitis shows certain ultrosonographic characteristics. All the cases were verified through surgery or follow-up visits. Conclusion： color coppler ultrasound is of great value in diagnosis of acute appenticitis which proves higher in accuracy and can initially suggests the types of appenticitis. It is the preferred method for imaging diagnosis which can assist in the clinical diagnosis of acute appenticitis and the choice of treatment plan.
