经腹股沟腹膜前间隙UPP三维网片治疗腹股沟疝临床分析

目的:研究开放式经腹股沟入路腹膜前间隙UPP三维网片无张力疝修补手术临床效果。方法:30例采用疝环充填式无张力疝修补术(Rutkow术式),26例采用腹膜前间隙UPP修补术。比较两种手术方法在手术时间、手术后疼痛、住院时间、住院费用、恢复工作时间、术后并发症和复发率的差别。结果:两组患者在恢复工作时间、术后并发症和复发率差异均无统计学意义(P0.05)。在住院费用上Rutkow组低于UPP组(P0.01)。在手术后疼痛、术后异物感方面Rutkow组明显高于UPP组(P=0.002)。结论:两种无张力疝修补术治疗单侧原发性腹股沟疝疗效肯定,UPP术术后异物感轻,对术后生活质量要求高者,建议用UPP术式。

Transinguinal preperitoneal groin hernia repair technique using a partially adsorbable three-dimensional preperitoneal mesh

Objective： To compare （he clinical effects of Rutkow, UPP procedures on inguinal hernia repair. Methods ： Thirty patients received mesh plug tension free hernioplasty （Rutkow procedure） while the other twenty six patients suffered the transinguinal preperitoneal groin hernia repair uses a partially adsorbable three-dimen- sional preperitoneal mesh named UPP procedure.The operation time, postoperative pain, time of hospital stay, time to return to work, cost, postoperative complication and recurrent rate were compared among the two groups. Re- sults ：There was no significant difference in postoperative pain, time of hospital stay, time to return to work, postop- erative complication and recurrent rate among the three groups （P〉0.05）. However, Rutkow group had a significant- ly lower cost than UPP （P〈0.01）. Rutkow group had a higher incidence of foreign body sensation than UPP （P= 0.002）. Conclusions： The two types of tension-free hernioplasty are all effective in treatment of primary monolateral inguinal hernia. UPP hernioplasty is the choice procedure for repair of primary inguinal hernia because of more comfortable while the patients have a good economic condition and pursuit a higher level on life quality.
