腹腔镜与开腹全子宫切除术的临床分析

目的:比较腹腔镜子宫切除术(TLH)及开腹全子宫切除术(TAH)的临床效果.方法:住院治疗的90例有全子宫切除指征患者分为腹腔镜手术组45例和开腹手术组45例,对比观察两组患者手术时间、术中出血量、肛门排气时间、术后平均住院日及切1:7愈合情况等.结果:腹腔镜手术组手术时间与开腹手术组比较差异无统计学意义(P>0.05),腹腔镜手术组术中出血量、肛门排气时间、术后平均住院日及切口甲级愈合情况等均明显优于开腹手术组(p<0.05).结论:腹腔镜手术组较开腹全子宫切除的优势明显,但两种术式不能完全替代,应根据患者情况、医生的技术水平和医院的条件综合考虑术式,以达到最佳治疗效果.

Laparoscopic versus abdominal total hysterectomy:a clinical analysis

Objective： To compare the total laparoscopic hysterectomy （TLH）, total laparoscopic hysterectomy and abdominal hysterectomy with resection （TAH）, the clinical effect.Methods： 90 cases of hospitalization full hysterectomy indications were divided into 45 cases of laparoscopic surgery group and 45 cases of open surgery group, compared the two groups were observed operation time, blood loss, flatus, the average postoperative length of stay and wound healing. Results： The laparoscopic surgery operative time and the open surgery group was no significant difference （P 〉 0.05）, intraoperative blood loss of laparoscopic surgery group; anal exhaust time, the mean postoperative length of stay and the incision Grade healing and so much better than the open surgery group （P〈0.05）. Conclusions： Laparoscopic surgery group compared with abdominal hysterectomy advantage of obvious, but the two procedures can not entirely replace, should consider the surgical patient＇s condition, the technical level of doctors and hospital conditions in order to achieve the best therapeutic effect.
