摘    要:目的探讨改良抗酸染色与解读法在病理诊断结核病(tuberculosis,TB)中的应用。方法应用H.E和改良Z.N抗酸染色法,从典型TB病变中寻求抗酸杆菌及其片段的判定标准,应用于不典型病例的诊断并随访结果。结果典型TB 30例中,典型抗酸杆菌阳性12例(40%),短杆菌阳性27例(90%)。不典型TB 66例中,典型杆菌阳性33例(50%),短杆菌全部阳性,经抗结核均获良好疗效,其中经手术的10例均发现典型TB病变。结论在临床和病理学基础上,应用改良抗酸染色与解读法在杆菌片段的背景下确认短杆菌可将典型TB细菌学阳性率和不典型TB诊断率均提高50%,具有显著的临床价值。

Abstract:Objective To explore to used of reformative antacid staining and its unscramble in pathological diagnosis with tuberculosis(TB).Methods Used of hematoxylin-eosin(H.E) and reformative Ziehl-Neelsen(Z.N) methods for coloring,in order to study a standard on the acid-fast bacillus and its segment were confirmed from 30 cases of TB with typical pathological change,and the results were used to in diagnosis with non-typical cases of TB and call on their results.Results In the typical cases,typical antacid bacillus was found 12 of 30,positive rate was 40%,and the short bacillus was found 27 of 30,positive rate was 90%.In the non-typical cases,typical bacillus was found 33 of 66,positive rate was 50%,and the short bacillus was found and achieved a good effect of anti-TB in all cases,and they had found typical pathological change with TB in all of 10 cases after the operation.Conclusion It have significant clinical value that can lead both is increased 50% that positive rate of bacteriology with typical TB and diagnosis rate of non-typical TB that short bacillus is identified under the background with the bacillus segments by use of reformative antacid staining and its unscramble methods on the base of pathology and clinical data.
