胆囊切除术中胆道造影168例临床分析

目的探讨胆囊切除术中胆道造影在胆石症病人的指征和临床实用价值。方法对我院自1999年1月￣2006年2月收治的168例胆囊切除术中胆道造影临床资料进行回顾性分析。结果168例均经胆囊管插管或胆总管直接穿刺术中胆道造影成功,其中发现胆总管结石27例;胆管损伤2例;胆囊管解剖异常3例;假阳性3例,假阴性4例。准确率为95.83%。结论胆囊切除术中合理选择应用胆道造影可提高阳性胆管结石诊断率,减少胆管结石残余发生率,降低胆总管探查阴性率,预防和及时发现胆管损伤,是一种安全、简单、有效的检查手段。

Clinical analysis of 168 cases with cholangiography in cholecystectomy

Objective To evaluate the indications and application value of intraoperative cholangiography during cholecystectomy. Methods The clinical data of intraoperative cholangiography of 168 cases were retrospectively analyzed from January 1999 to February 2006 in our hospital. Results 168 patients underwent transcystic intraoperative cholangiography or direct injection into the common bile duct at operative cholangiography,all were successfully completed. Among these cases,Choledocholithiasis were detected in 27,trauma of biliary tract were detected in 2,abnormal cystic duct were detected in 3,false-postive were detected in 3,false negative were detected in 4. The accuracy rate was 95.83%. Conclusions Cholangiography in cholecystectomy should be used reasonably,which would increase the diagnostic accordance rate of cholelithiasis,reduce the occurrence of post-operative residual calculi,reduce the negative rate of choledochus incision exploration,and prevent trauma of biliary tract. So it was a safe,simple and effective way.
