中医辨证分型高血压病病人颅内动脉脑血流动力学变化

目的观察不同中医证型高血压病病人的颅内动脉脑血流动力学变化。方法全部病例在治疗前接受中医辨证分型和经颅多普勒超声检查。结果病例组中出现颅内各动脉血流异常病人46例，对照组为9例。不同中医证型高血压病病人颅内各动脉血流异常率为阴虚阳亢证9％，阴阳两虚证8％。痰湿壅盛证8％，肝火亢盛证3％。肝火亢盛证和阴虚阳亢证等证型高血压病病人的大脑中动脉（MCA）、大脑前动脉（ACA）、大脑后动脉（PCA）、颈内动脉终末端（ICA）和椎动脉（VA）等指标明显高于阴阳两虚证组。对照组MCA、A（A、PCA、ICA、VA和BA等指标均明显低于不同中医证型高血压病病人（P〈0．05）。结论各中医证型高血压病病人常有颅内动脉血流速度和脉动指数指标异常表现，特别是阴阳两虚证病人。

Cerebral Arterial Hemodynamics in Patients with Hypertension of Different Dialectical Types of Traditional Chinese Medicine

Objective To investigate structure of left ventricle and cerebral arterial hemodynamics in patients (pts) with hypertension by different dialectical types of traditional Chinese medicine (TCM).Methods The data of dialectical type by TCM and cardiac function by Doppler ultrasonic cardiogram were collected before treatment in all pts. Results One hundred and five pts with primary hypertension were as follow by TCM:Twenty-four pts with florid anger (22.9%), 37 with weak yin and strong yang (35.3%), 16 with wet sputum and florid emphraxis (15.2%) and 28 with weakness both yin and yang (26.7%). MCA, ACA, PCA, ICA, VA and BA in the control group were obviously higher than that in pts with hypertension by TCM (P<0.05).MCA,ACA,PCA,ICA,VA and BA in the control group were lower than that in pts with different type by TCM (P<0.05).Conclusion Cerebral arterial blood flow rate and arterial hitting index were abnormal in pts with different TCM dialectical type hypertension, especially in pts with weakness of yin and yang. When blood pressure was controlled, it was possible that blood circulation would improved and stroke complication would reduced by dialectical treatment.
