胸部爆震伤3例救治分析及文献复习

目的 探讨胸部爆震伤的临床特征及治疗策略。方法 报道3例胸部爆震伤患者临床资料,并检索复习相关文献。结果和结论 3例患者均为青年男性,因“爆炸致伤3小时”入院,诊断为胸部爆震伤,经正确的外科处置、合理的综合治疗及积极的多发伤、复合伤救治,3例患者胸部损伤完全康复。文献复习显示,根据冲击波、弹片等致伤因素的不同,爆震伤可分类为四级。胸部爆震伤损伤严重、伤情复杂,可表现为“外轻内重”、全身反应严重,需从现场急救、手术治疗、综合治疗、多发伤及复合伤救治等方面开展合理救治。

Treatment of blast thoracic injuries: three cases report and literatures review

Objective To explore the clinical characteristics and treatment of blast thoracic injurie due to firearm.Methods The diagnosis and treatment of 3 patient with blast thoracic injuries were reported, and the relevant literatures were reviewed. Results and Conclusion The 3 patients were all young males and were admitted to the hospital due to blast injuries for 3 hours. They were diagnosed with blast thoracic injury and recovered after effective surgical treatment, reasonable comprehensive treatment and active treatment for multiple injuries or compound injuries. Literature reviews suggested that blast injury can be classified into four levels according to different injury factors such as shock wave and shrapnel. Blast thoracic injuries are serious and complicated, which can be manifested as seemingly slight but actually severe and serious systemic reaction. Reasonable treatment strategies should include on-site first aid, surgical treatment, comprehensive treatment, and treatment of multiple or compound injuries.
