肝脏小血管瘤的螺旋CT四期表现及评价

目的研究肝脏小血管瘤螺旋CT四期表现并进行评价。方法40例经手术和MR检查及病理确诊的肝脏小血管瘤进行回顾分析。结果40例患者共发现48个病灶,67%(32/48)具有公认典型血管瘤强化表现,25%(12/48)通过四期表现分析可以明确诊断,8%(4/48)四期扫描不能确诊。平扫48个病灶均为低密度。增强后动脉期扫描32(67%)个病灶边缘部强化,12(25%)个均匀显著强化,4(8%)个病灶不强化。门脉期32(67%)个病灶均匀强化,12(25%)个病灶强化范围由边缘向中央部扩大,4(8%)个病灶不强化,平衡期和延迟期44(92%)个病灶呈等密度,4(8%)个病灶不强化。结论肝脏小血管瘤的螺旋CT四期增强扫描表现具有典型和非典型改变,认识这些表现,可以与肝脏的其他常见占位性病变鉴别。

Small Hepatic Hemangioma: an evaluation of 1-phase Spiral CT findings

Objective To study and evaluate the 4-phase spiral CT manifestations of the hepatic small hemangioma.Methods 40 cases hemangioma,diagnosed by operation or CT or MRI,were analyzed retrospectively.Results There were 48 lesions in all the 40 patients,67% of these had typical enhanced aspects,25% were diagnosed by 4 -phase enhanced scanning,besides 8% atypical cases.In unenhanced scan,47 lesions were homogenous hypo-attenuation in hepatic parenchyma,and 1 lesion was equal with hepatic parenchyma.After the contrast media was injected,the peripheral enhancement was revealed in 32 lesions in arterial phase and completed enhancement in 12 lesions,while unenhancement in 4 lesions.In portal venous phase the enhancement involved the tumors from periphery to the center was revealed in 32 lesions and completed enhancement in 12 lesions,while unenhancement in 4 lesions.In delayed images,filled-in was completed in 44 lesions,while unenhancement was in 4 lesions.Conclusion The four-phase spiral CT manifestations of the hepatic small hemangioma were typical and atypical characteristic,which may help differentiating the hepatic small hemangioma from other liver neoplasms.
