减重代谢手术治疗肥胖型OSAS的应用现状

肥胖是阻塞性睡眠呼吸暂停综合征(OSAS)最重要的可逆性危险因素，40%的肥胖患者同时遭遇OSAS，而70%的OSAS患者均属于肥胖人群 ^{1 ]}。OSAS的治疗中，2013版的美国《OSAS规范诊疗指南》 ^{2 ]}中明确指出，强烈建议肥胖型OSAS病人进行减重治疗，减重代谢手术目前作为肥胖症唯一且长期有效的治疗手段 ^{3 ]}，不仅治疗肥胖，对于肥胖合并症如OSAS、2型糖尿病、高血压和高血脂等代谢紊乱性疾病也具有显著疗效，其中OSAS的治愈高达84%，缓解率可达86% ^{4 ]}。但其治疗肥胖型OSAS的机制尚未明确，手术适应证及手术方式的选择仍未有统一共识参考，为此综述减重代谢手术治疗肥胖型OSAS的应用现状。
