机械通气联合血液灌流抢救急性重度有机磷农药中毒

目的:探讨机械通气联合血液灌流对急性重度有机磷农药中毒的临床疗效。方法:40例患者分为治疗组19例和对照组21例,对照组为机械通气和常规治疗,治疗组在对照组基础上联合血液灌流。观察两组机械通气时间、胆碱酯酶恢复正常所需时间、阿托品用量及多器官功能障碍综合征(MODS)、中间综合征(IMS)发生率和治愈率。结果:与对照组相比,治疗组治愈率明显提高、MODS及IMS发生率减少、机械通气时间和胆碱酯酶恢复正常所需时间明显缩短、阿托品用量明显减少(P<0.05或P<0.01)。结论:在常规治疗的基础上,机械通气联合血液灌流对急性重度有机磷农药中毒疗效显著。

The treatment of a combination of mechanical ventilation and hemoperfusion in rescuing acute severe organophosphoruspesticide poisoning

objective: To investigate the clinical effect of a combination of mechanical ventilation and hemoperfusion in rescuing acute severe organophosphorus pesticide poisoning. Method: Forty patients with acute severe organophosphorus pesticide poisoning were randomly divideded into study group (n=19) and control group (n=21). The control patients were commenced on mechanical ventilation in addition to the conventional therapy, the study patients were combined with hemoperfusion on the base of the control group. Observe the duration of mechanical ventilation, the required time of cholinesterase returned to normal, dosis of atropine, the incidence of intermediate syndrome (IMS) and multiple organ failure syndrome (MODS), the cure rate. Result: Compared with control group, the cure rate elevated obviously, the incidence of IMS and MODS reduced, the duration of mechanical ventilation and required time of cholinesterase returned to normal were shortened obviously, dosis of atropine was reduced significantly in the study group (P<0.05 or P<0.01). Conclude: A combination of mechanical ventilation and hemoperfusion in rescuing acute severe organophosphorus pesticide poisoning has notable effect on the base of conventional therapy.
