摘    要:目的探讨长程心率变异性分析在心脏神经官能症患者中的临床意义。方法采用动态心电图 ,结合时域和频域的方法 ,对 48例神经官能症患者和 45例正常者进行长程心率变异 (HRV)分析。结果① 2 4小时、白天的RMSSD、PNN 50观察组均低于对照组(P <0 .0 5或 <0 .0 1 ) ;夜间两组HRV比较无差异 (P >0 .0 5)。②观察组与对照组的RMSSD、PNN 50、HF白天均低于夜间(P <0 .0 1或 <0 .0 5)。结论①心脏神经官能症患者的迷走神经活性减低 ,且以白天为主 ,夜间则减低不明显。②心脏神经官能症患者HRV与正常人一样存在昼夜节律。临床可根据上述特点鉴别心脏神经官能症与器质性心脏病。

Abstract:ObjectiveTo investigate the clinical significance of long-term heart rate variability(HRV)of patients of cardiac neurosis dysfunction. Methods Long-term heart rate variability in 48 patientswith cardiac neurosis and 45 healthy patients was analyzed by application of ambulatoryelectrocardiogram combined with time and frequency domain. Results 24 h, daily rMSSD andPNN50 of the observation group were lower than that of the control group(P<0.05 or P<0.01).Night HRV in the two groups were not different(P>0.05). Daily rMSSD, PNN50 and HF in the two groups were lower than that in the night (P<0.01 or P<0.05). Conclusion The activities of pneumogastric nerve of patients of cardiac neurosis dysfunction declines, especially in the daytime while declines not clearly in the evening. HRV of patients of cardiac neurosis dysfunction hasthe same rhythm of day and night as that of normal people. Cardiac neurosis dysfunction andorganic cardiac disease can be distinguished on the basis of above-mentioned characteristics.
