胃癌的癌胚抗原、雌激素、孕激素受体测定

使用免疫组化ＳＰ法测定３３例胃癌组织的癌胚抗原（ＣＥＡ）、雌激素受体（ＥＲ）、孕激素受体（ＰＲ）。ＣＥＡ阳性率５４．５％（１８／３３）、ＥＲ阳性率４８．５％（１６／３３）、ＰＲ阳性率３３．３％（１１／３３）。ＣＥＡ阳性率与ＥＲ阳性率；在高分化肿瘤ＣＥＡ与ＥＲ阳性表达；在中低分化肿瘤ＣＥＡ与ＥＲ阳性表达；ＰＲ阳性率与ＥＲ阳性率；均无显著性差异（Ｐ值均大于０．０５），说明三者之间存在相关性，推测可能存在相类似的表达机制，ＰＲ为ＥＲ具有生物学特性表现。ＣＥＡ、ＥＲ阳性多分布于中低分化肿瘤，与高分化肿瘤之间存在显著性差异（ＣＥＡ：Ｐ＜０．０１；ＥＲ：Ｐ＜０．０５），说明它们的表达与癌细胞分化程度呈负相关。ＣＥＡ、ＥＲ阳性胃癌预后及５年生存率差于阴性者。但化疗可改善之，故测定ＣＥＡ、ＥＲ、ＰＲ可作为推测胃癌预后的参考指标，指导临床对这些表达阳性的胃癌患者进行化疗及内分泌治疗，改善预后

The Determination of Receptors of CEA,ER and PR of Gastric Carcinoma

SP immunohistochemical technique was applied to determine carcinoembryonic antigens (CEA),estrin receptors (ER) and progestogen receptors (PR)on the cancerous tissues of 33 cases of gastric cancer.The positive rates of CEA,ER,PR were respectively 54 5%(18/33),48 5%(16/33)and 33 3%(11/33).The positive rates of CEA and ER,the positive expressions of CEA and ER of the well differentiated carcinoma,the positive expressions of CEA and ER of the poor differentiated carcinoma,the positive rates of PR and ER had no signifficant differences ( P >0 05)which showed that there were no correlations among the three indices.It was estimated that there was similar expression mechanism among them and that PR indicated the biological characters of ER.CEA and ER with majority and were positive in the poor differentiated tumors,which were signifficently different from well differentiated ones (CEA: P <0 01;ER: P <0 05).Their expressions had negative correlation with the degree of cancerous cells.The prognosis and 5 year survival rate with CEA (+) and ER (+) were worse than those with CEA (-) and ER (-).So CEA,ER and PR can be used as reference indices of the prognosis,chemotherapy and endocrinotherapy of gastric cancer.
