胸部Castleman病MSCT影像表现

目的通过分析Castleman病的多层螺旋CT(MSCT)表现,增强对该病的认识,降低临床误诊率。方法回顾性分析8例经病理证实的Castleman病的临床资料和MSCT表现,与病理结果进行对照研究。结果病变部位:中纵隔或肺门者6例,前纵隔和后纵隔各2例。临床分型:局灶型7例,多中心型1例。病理分型:透明血管型7例,浆细胞型1例。MSCT表现:MSCT平扫示病灶为等密度,2例伴有钙化;增强扫描示透明血管型病灶有明显强化,浆细胞型病灶呈不均匀强化。结论 Castleman病的CT表现与病理类型密切相关,透明血管型病变的MSCT表现具有特征性。

Mufti-slice spiral CT findings of Castleman disease

Objective To study the CT findings of Castleman＇s disease and the diagnostic value of CT for Castleman＇s disease. Methods Clinical data and CT findings of Castleman＇s disease proved by pathology in 8 cases were retrospectively analyzed and correlated with pathology findings. Results Locations of the lesions were as follows： middle mediastinal or hilus of lung（ n = 6）,anterior and posterior mediastinum（ n =2）. The clinical subtypes included localized type in 7 cases and multi- centric type in 1 cases. As for histopathology classification,there were hyaline- vascular type in 7 cases and plasma cell type in 1 case. Apart from 2 cases with calcifications,the other cases showed homogeneous attenuation on plain CT. After contrast administration,marked and sustained enhancement was showed in hyaline vascular type,mild- moderate enhancement was showed in plasma cell type. Conclusion CT findings of Castleman＇s disease were closely correlated with pathological subtypes. CT features of hyaline- vascular subtype are characteristic.
