益生菌联合麦芽糊精果糖肠内营养对胃癌患者术后的影响

摘    要:目的 研究添加益生菌联合麦芽糊精果糖肠内营养对胃癌患者术后恢复情况及主观舒适度、腹胀发生情况的影响。方法 研究对象采用徐州医科大学附属连云港医院2016年6月至2019年6月收治的择期手术胃癌患者120例,采用随机数字表分为两组,分别为对照组和试验组,每组患者60例,两组患者均进行胃癌D2根治术,手术后两组患者均采取SOX方案进行化疗。试验组患者术后采取添加益生菌联合麦芽糊精果糖肠内营养辅助性治疗。对照组患者采取添加益生菌的肠内营养治疗。治疗时间均为2周。观察两组患者的肠道微生态、营养状况、主观舒适度以及不良反应之间的差异。结果 治疗后,两组患者的粪便乳酸杆菌、双歧杆菌、肠球菌、真杆菌均有所升高,且试验组乳酸杆菌、双歧杆菌、肠球菌、真杆菌菌落数明显高于对照组,其差异具有统计学意义(P<0.05)。两组患者的HB、HCT、TBIL、DBIL、IBIL、TP、ALB、PAB均有明显升高(P<0.05),且试验组患者的HB、HCT、TP、ALB、PAB、ALT明显高于对照组(P<0.05),TBIL、DBIL、IBIL明显低于对照组(P<0.05),两组患者的主观舒适度评分均显著下降,且试验组患者的主观舒适度评分显著小于对照组,试验组患者的腹泻、腹胀以及呕吐等情况发生率显著低于对照组,差异存在统计学意义(P<0.05)。结论 益生菌联合麦芽糊精果糖肠内营养对胃癌患者术后使用,患者的主观舒适度以及营养状况均显著改善,腹胀、腹泻以及呕吐发生率显著下降,患者临床症状改善显著,建议临床推广。

Effect of probiotic enteral nutrition support combined with maltodextrin fructose on postoperative patients with gastric cancer

Abstract:Objective To study the effects of probiotic enteral nutrition support combined with maltodextrin fructose drink on the recovery,subjective comfort and abdominal distension of patients with gastric cancer after operation.Methods 120 patients with gastric cancer who underwent elective surgery in Lianyungang hospital of Xuzhou Medical University from June 2016 to June 2019 were divided into two groups by random number table:control group and observation group,60 cases in each group.Both groups underwent D2 radical gastrectomy for gastric cancer.After operation,both groups received SOX regimen for chemotherapy.The patients in the observation group were treated with probiotic enteral nutrition support combined with maltodextrin fructose beverage after operation.The patients in the control group reccived enteral nutrition support with probiotics.The treatment lasted for 2 wecks.The differences of intestinal microecology,nutritional status,subjective comfort and adverse reactions between the two groups were observed.Results After treatment,the number of Lactobacillus faecalis,Bifidobacterum,Enterococcus and Eubacterium in the two groups both increased,and the number of Lactobacillus,Bifidobacterium,Enterococcus and Eubacterium in the observation group was significanly higher than those in the control group(P<0.05).The levels of HB,HCT,TBIL,DBIL,IBIL,TP,ALB and PAB in the two groups were significantly increased(P<0.05),and the levels of HB,HCT,TP,ALB,PAB and ALT in the observation group were significantly higher than those in the control group(P<0.05),while the levels of TBIL,DBIL and IBIL were significantly lower than those in the control group(P<0.05).The subjective comfort score of the observation group was significantly lower than that of the control group,and the incidence of diarrhea,abdominal distension and vomiting of the observation group was significantly lower than control group(P<0.05).Conclusions Probiotic enteral nutrition support combined with maltodextrin fructose beverage can significantly reduce the incidence of abdominal distension,diarrthea and vomiting in patients with gastric cancer after operation.It is suggested that clinical application of probiotic enteral nutrition support combined with maltodextrin fructose beverage should be promoted.
