腮腺部分切除术治疗腮腺多形性腺瘤的临床疗效分析

【目的】探讨保留腮腺主导管的腮腺部分切除术在治疗肿瘤直径小于3．0cm的腮腺多形性腺瘤（PAG）的可行性。方法]36例PAG随机分为三组，其中16例采用腮腺部分切除术（A组），12例行腮腺浅叶切除术（B组），8例行腮腺全切除术（C组），随访时间3-5年。【结果】A组术后瘢痕和面部凹陷畸形、味觉出汗综合征（Frey综合征）、暂时性面瘫发生及腮腺功能等方面均优于B、C组，肿瘤的复发无明显差异。【结论】腮腺部分切除手术创伤小，并发症少，还可以保存腮腺一定的功能，是PAG临床治疗的理想术式。

Clinical efficacy of Partial Parotidectomy for the Treatment of Pleomorphic Adenoma of Parotid

Objective]To investigate the feasibility of preserving parotid duct in partial parotidectomy for the treatment of pleomorphic adenoma of parotid gland with less than 3.0cm in diameter. Methods]Thirty six patients with pleomorphic adenoma of parotid gland were randomly divided into three groups. Group A with 16 patients received partial parotidectomy. Group B with 12 patients received conventional superficial lobe operation. Group C with 8 patients received total parotidectomy. All patients were followed up for 3-5 years. Resuits] In Group A, the incidence of postoperative scar, facial depression deformity, Frey＇s syndrome, temporary facial paralysis and the parotid function were superior to Group B and C, but there was not obvious difference in tumor recrudescence. Conclusion]The partial parotidectomy is a minimally invasive surgical method with few complications. It can be thought as the ideal choice for clinical treatment of pleomorphic adenoma of parotid gland.
