妊娠期糖尿病病人脐血瘦素水平与母血瘦素水平及新生儿体质量的关系

①目的　探讨妊娠期糖尿病 (GDM )病人新生儿脐血瘦素水平与母血瘦素及新生儿体质量的关系。②方法　用放射免疫方法测定 30例GDM孕妇血清和其新生儿脐血瘦素浓度及 2 4例正常孕妇脐血瘦素浓度 ,同时测量新生儿的体质量、胎盘质量、孕妇体质量的增加情况等。③结果　GDM孕妇血清瘦素水平明显高于新生儿脐血 (t=3.5 1,P <0 .0 0 1) ,但二者无相关性 ;GDM孕妇血清瘦素水平与体质量指数 (BMI)和孕期体质量的增加呈正相关 (r=0 .5 83、0 .4 13,P <0 .0 5 ) ;GDM孕妇新生儿脐血瘦素水平与新生儿出生体质量和胎盘质量呈正相关(r =0 .5 0 8、0 .4 84 ,P <0 .0 5 ) ;与正常孕妇新生儿脐血瘦素水平相比 ,GDM孕妇新生儿脐血瘦素水平无明显增高(t=0 .6 5 3,P >0 .0 5 )。④结论　GDM孕妇新生儿脐血瘦素水平明显低于母血瘦素水平 ,且与新生儿体质量及胎盘质量呈正相关关系 ,有望成为预测胎儿体质量的指标之一

RELATIONSHIP BETWEEN UMBILICAL BLOOD LEPTIN LEVELS,MATERNAL LEPTIN LEVELS AND NEONATAL BIRTH WEIGHT IN PATIENTS WITH GESTATIONAL DIABETES

Objective To investigate the relationship between umbilical blood leptin levels, and maternal serum leptin levels, and neonatal birth weight in patients with gestational diabetes mellitus(GDM). Methods Serum leptin levels of 30 GDM and their neonates'umbilical blood and serum leptin levels of 24 of normal pregnancy women were measured with RIA. Neonatal birth weight, placental weight and the changes of all the pregnant women's weight were measured. Results The mean leptin concentration of the GDM patients was significantly higher than that of the neonatal umbilical blood(t=3.51, P<0.001). In the GDM patients, there was a positive correlation between maternal leptin levels, BMI and pregnant weight gain(r=0.583, 0.413; P<0.05). A positive correlation was found between umbilical blood leptin levels, birth weight and placental weight in the newborns of GDM pregnant women (r=0.508,0.484;P<0.05). There were no significant differences between leptin levels of newborns of normal mothers and GDM patients (t=0.653,P>0.05). Conclusion Maternal leptin levels are significantly higher than umbilical blood leptin levels in the GDM patients. A positive correlation exists between umbilical blood leptin levels, birth weight and placental weight in the newborns of GDM pregnant women, which suggests that leptin could be another index to measure newborn weight.
