Impact on peritonsillar infections and microflora of phenoxymethylpenicillin alone versus phenoxymethylpenicillin in combination with metronidazole |

Abstract: | Summary In a double-blind study, 20 patients with peritonsillar abscesses were treated with 2 g phenoxymethylpenicillin b.i.d. for ten days together with needle aspiration, incision and daily drainage, and 20 patients were treated with 2 g phenoxymethylpenicillin b.i.d. and 0.8 g metronidazole b.i.d. for ten days together with needle aspiration, incision and daily drainage. Group A beta-hemolytic streptococci were isolated from pus in 20 of the patients with peritonsillar abscesses, in five of these together with indigenous oropharyngeal aerobic and anaerobic microorganisms. Pure anaerobic bacteria were found in nine abscesses, together with indigenous aerobic microorganisms in eight, and together with group A, C and G streptococci in five. In one patient heavily colonized with beta-lactamase-producingStaphylococcus aureus, Haemophilus parainfluenzae andBacteroides, group A beta-streptococci failed to be eradicated. In the penicillin group, nine of 18 patients harboured beta-lactamase producingBacteroides strains in the tonsils on the day of admission. On the third and tenth days of treatment all patients harboured beta-lactamase producingBacteroides strains in the tonsils, while in the penicillin + metronidazole group, only one out of 17 patients still harboured beta-lactamase producingBacteroides strains. None of the patients harboured beta-lactamase producing fusobacteria on the day of admission. In the penicillin group, however, beta-lactamase producing fusobacteria were recovered from three patients on the tenth day of treatment. No beta-lactamase producing fusobacteria were recovered from the penicillin + metronidazole group. No clinical differences were noted between the patient group treated with phenoxymethylpenicillin alone and that treated with phenoxymethylpenicillin in combination with metronidazole, and no clinical relapses were observed, indicating that surgical drainage is an important part of treatment.
Vergleichsstudie zur Beeinflussung peritonsillärer Infektionen und der Mikroflora durch Phenoxymethylpenicillin allein und in Kombination mit Metronidazol Zusammenfassung Im Rahmen einer Doppelblindstudie wurden 20 Patienten mit Peritonsillarabszeß mit 2 g Phenoxymethylpenicillin 2 ×Tag und Nadelaspiration, Inzision und täglicher Drainage behandelt; bei 20 Patienten wurde neben Nadelaspiration, Inzision und täglicher Drainage eine Therapie mit 2 g Phenoxymethylpenicillin plus 0,8 g Metronidazol 2 ×/Tag durchgeführt. Die Therapiedauer war bei beiden Gruppen auf zehn Tage festgelegt. Bei 20 Patienten mit Peritonsillarabszeß wurden aus dem Eiter -hämolysierende Streptokokken der Gruppe A isoliert, bei fünf von ihnen zusätzlich aerobe und anaerobe Mikro-organismen der endogenen oropharyngealen Flora. In neun Abszessen fanden sich anaerobe Bakterien in Reinkultur, in acht Fällen wurden zusätzlich endogene aerobe Keime und in fünf Fällen Streptokokken der Gruppen A, C und G isoliert. Bei einem Patienten mit starker Besiedelung durch -lactamasebildendeStaphylococcus aureus, Haemophilus parainfluenzae undBacteroides-Stämme war eine Eliminierung der -hämolysierenden Streptokokken der Gruppe A nicht erfolgt. In der Penicillin-Gruppe waren am Einweisungstag bei neun von 18 Patienten -lactamasebildendeBacteroides-Stämme in den Tonsillen nachzuweisen. Am dritten und zehnten Behandlungstag fanden sich bei allen Patienten -lactamasebildendeBacteroides-Stämme in den Tonsillen, in der Penicillin + Metronidazol-Gruppe konnten dagegen nur noch bei einem von 17 Patienten -lactamasebildendeBacteroides-Stämme angezüchtet werden. Am Tag der Einweisung waren bei keinem Patienten -lactamasebildende Fusobakterien nachzuweisen. Amzehnten Behandlungstag fanden sich dagegen in den Kulturen von drei Patienten -lactamasebildende Fusobakterien. In der Penicillin + Metronidazol-Gruppe ließen sich keine -lactamasebildenden Fusobakterien isolieren. Im klinischen Therapieerfolg unterschieden sich mit Phenoxymethylpenicillin allein behandelte Patienten nicht von Patienten, die die Kombination Phenoxymethylpenicillin + Metronidazol erhalten hatten; klinische Rezidive wurden nicht beobachtet, was dafür spricht, daß die chirurgische Drainage ein wichtiger Teil der Behandlung ist. |
