巨大岩斜区肿瘤的显微外科治疗(附15例报告)

目的：报告１５例巨大岩斜区肿瘤的显微外科治疗。方法：分析１５例巨大岩斜肿瘤的临床及影像学特征、手术方法、手术结果、术后并发症等。结果：肿瘤全切除１３例，近全切除２例，无手术死亡，５例术后颅神经损害症状加重，２例无任何颅神经损伤症状，８例颅神经症状同术前。结论：巨大岩斜肿瘤采用幕上下经岩骨乙状窦前入路可全切除肿瘤，熟悉该入路的显微解剖知识可降低术后并发症和颅神经损伤率。

Microsurgical treatment of giant petroclival tumors

Objective: To discuss the microsurgical treatment of 15 cases with giant tumor in petoclival region. Methods: The clinical and preoperative MRI features, operative approaches, result of operation postoperative complications and care of giant petroclival tumors were discussed in 13 patients. Result: Total resection of the tumor was achieved in 13 cases, subtotal resection in 2 case. No patient died in this series. 5 patients had worsening CNs palsies, 2 patients had intact, and 8 potients CNs deficit did not change after operation. Conclusion: The giant tumors in petroclival region can be totally resected by transpetrosal presigmoid approach. Understanding of the microanatomy about this approach can greatly reduce postoperative morbidity of patients.
