开放性前颅凹底骨折的治疗

目的探讨开放性前颅底骨折的诊断与治疗方法，容易产生的并发症及手术效果。方法住院治疗的符合开放性前颅底骨折诊断标准的患者81例，分别予以脑脊液鼻漏修补术12例，颅骨修补术12例，清除颅内血肿或挫伤灶同时行硬脑膜修补术的24例，眶顶骨性重建3例，行保守治疗30例。结果良好69例，中残6例，重残4例，因颅内感染死亡2例。结论对开放性前颅凹底骨折治疗的关键是做好脑部清创术，清除可能引起颅内感染的因素，对脑脊液鼻漏及时、恰当的处理，同时要兼顾功能与整容的目的。

Treatment of Open Anterocranial Fossa Fracture

Objective To approach a method of diagnosis and treatment on open anterocranial fossa fracture, and its complication, and surgical effect. Methods 81 patients that fitted to diagnostic code of opening anterocranial fossa fracture were operated. 12 cases were enforced restore operation of the cerebrospinal rhinorrhea. 12 cases were skull osteoplasty. 24 cases were clean up intracranial hematomas or brain contend focus, at the same time the cerebral dura mater were did neoplasty. 3 cases orbital roof was re - established with the bone. 30 cases were expectant treatment. Results 69 cases were good, 6 cases were mid - handicap. 4 cases were re - handicap. 2 cases died. Conclusion The key point of treatment of opening anterocranial fossa fracture is wound excision of pars encephalica, the factor is removed that cause intracranial infection, the cerebrospinal rhinorrhea is treated promptly, and to give consideration to function and face -lifting.
