CT引导下乙醇注射加椎旁置管腰交感神经节毁损术疗效评估

摘    要:目的 评估CT引导下单次乙醇注射加椎旁置管行腰交感神经毁损术对治疗下肢缺血性疾病的疗效.方法 L2交感神经节单次乙醇注射25例(B组);L2交感神经节单次乙醇注射加L3椎旁留置导管毁损治疗23例(BT组).全部病例均在CT引导下操作,术后3 d采用视觉模拟评分(VAS)法评估患者疼痛程度.BT组患者如第3天VAS仍然≥4分,则在患者俯卧位下首先经留置导管注入1%利多卡因3 ml,如5 min后VAS≤3,则再次经导管注入无水乙醇5 ml.1周后再次进行疼痛程度评估.结果 B组显效率32%,有效率80%.BT组显效率60.9%,有效率95.7%,两组间差异有统计学意义(P<0.01).结论 CT引导下乙醇注射加椎旁置管腰交感神经节毁损术明显优于单次乙醇注射毁损法,该方法 使治疗更加个体化,疗效更高,更易被医患接受.
