沙利度胺联合化疗治疗晚期胃癌疗效观察

目的 观察沙利度胺与化疗同步治疗晚期胃癌的疗效及毒副反应.方法 将78例晚期胃癌患者分为单纯化疗组、低剂量沙利度胺（50mg 2次/d）加化疗组及高剂量沙利度胺（100mg 2次/d）加化疗组,各26例.3组均于2个月后评价疗效.结果 单纯化疗组有效率42%,低剂量沙利度胺加化疗组有效率55%,高剂量沙利度胺加化疗组有效率59%,沙利度胺加化疗组有效率与单纯化疗组比较均有显著性差异（P均〈0.05）,且疗效与沙利度胺剂量无显著关系（P〉0.05）.3组毒副反应主要为便秘、嗜睡、乏力、末梢神经炎,其中高剂量沙利度胺加化疗组嗜睡及末梢神经炎发生率最为显著（P均〈0.05）.结论 沙利度胺联合化疗能提高近期有效率,而且低剂量沙利度胺未增加毒副作用.

Observation on thalidomide combined with chemotherapy in treatment of advanced gastric cancer

Objective It is to observe the curative effect and toxic reaction of thalidomide combined with chemotherapy in advanced gastric cancer. Methods 78 patients with advanced gastric cancer were randomly divided into three groups, 26 pa tients in lowdose group were treated with chemotherapy plus thalidomide （50mg bid） and 26 patients in highdose group with chemotherapy plus thalidomide （100mg bid） , the others in chemotherapy alone group with chemotherapy alone. The curative effect was evaluated after treatment of 2 months. Results The response rate was 42.3% in chemotherapy alone group, was 55.2% in lowdose group, and was 58.6% in highdose group. There were significant differences in effect rate of chemothera py alone group compared with the others （ all P 〈 0.05）. The effect rate was not correlated to the dose of thalidomide （ P 〉 0.05）. The main side effects were constipation, drowsiness, fatigue and peripheral neuritis in all groups, but the rates of drowsiness and peripheral neuritis in highdose group were higher than the others （ all P 〈 0.05）. Conclusion Thalidomide combine with chemotherapy can improve the curative effect of patients with advanced gastric cancer, and the toxic effects can be tolerated in lowdose.
