2型糖尿病和高血压病患者瘦素水平及其相关性分析

目的研究2型糖尿病、高血压病患者血清瘦素水平以及瘦素的影响因素。方法将215例患者分为糖尿病组（DM组）、高血压病组（EH组）、糖尿病合并高血压痛（DM＋EH）组，另设对照组54例，分别测定空腹血糖、糖化血红蛋白、空腹胰岛素（FINS）、总胆固醇、三酰甘油、高密度脂蛋白、低密度脂蛋白、瘦素水平，计算体质量指数（BMl）、胰岛素抵抗指数（HOMA—IR）。结果血清瘦素水平、HOMA—IR在DM＋EH组高于DM组、EH组，DM组、EH组高于对照组，瘦素与BMI、腹围、FINS、HOMA—IR、血压呈正相关，经多元回归分析，瘦素浓度的影响因素有BMI、腹围、HOMA—IR。结论2型糖尿病、高血压病患者血清瘦素水平升高，瘦素可能在2型糖尿病和高血压病的发病中起一定作用。

Leptin levels of patients with diabetes type 2 and hypertension and an analysis of their correlation

Objective It is to study serum leptin level and its effect factors in patients with diabetes type 2 and hyperten- tion. Methods The study included a total of 215 patients, and divided them into the following three groups： patients with dia- betes （group DM） , patients with hypertension （group EH） and patients with both diabetes and hypertension （group DM ＋ EH）. There was also a control group of 54 patients. For all the patients their levels of fasting plasma glucose, glycated hemo- globin, fasting insulin （FINS）, total cholesterol, triglycerides, high-density lipoprotein, low-density lipoprotein, leptin, body mass index （BMI） and Homeostasis Model of Assessment of Insulin Resistance （ HOMA - IR） were determined. Results Ser- um leptin level and HOMA - IR were higher in group DM ＋ EH than that in group DM and group EH, and higher in group DM and group EH compared to the control group. Leptin was positively correlated with BMI, abdominal circumference, FINS, HOMA -IR and blood pressure. After multiple regression analysis, factors affecting leptiu concentrations were BMI, abdomi- nal circumference and HOMA - IR. Conclusion Patients with diabetes type 2 and hypertension have an increased level of ser- um leptin. Leptin might to a certain degree cause diabetes type 2 and hypertension.
