陶瓷-陶瓷人工髋关节初步应用结果

目的　研究陶瓷型人工髋关节的中短期应用结果 ,以探讨这种假体的临床应用价值。方法　对人工全髋关节置换术 332例 345髋进行随访 ,其中陶瓷 -陶瓷非骨水泥型假体 31例 33髋 ,生物型固定超高分子聚乙烯 -钴铬钼合金全髋假体 30 1例31 2髋。应用Harris评分进行功能评价 ,应用股骨Gruen与髋臼Delee-Charnly分区方法进行影像学评价。结果　Harris评分两组患者之间未见显著性差异 ,但是影象学上 ,超高分子聚乙烯 -钴铬钼合金假体的松动范围或程度要大于或多于陶瓷 -陶瓷假体。结论　对于年轻患者 ,新型的陶瓷 -陶瓷假体中短期效果良好 ,影象学松动率明显低于超高分子聚乙烯 -钴铬钼合金假体。但是由于其价格昂贵 ,使用范围受到限制。而且远期结果有待于临床观察

The Preliminary Outcome of Ceramic on ceramic Hip Prostheses

Objective To study the result of application of ceramic on ceramichip prostheses and investigate the clinical value of it.Methods Three hundred and forty five hips in 332 cases of total hip arthroplasty were followed up. Thirty three hips in 31 cases were ceramic-on-ceramic hip prostheses, 312 hips in 301 cases were cobalt-chrome-molybednum on ultra high molecular weight polyethylene bearing surfaces. The post operative clinical results were evaluated with Harris hip scoring system. Radiographic analysis was got by Gruen's zonal method for the stem and the zonal classification of Delee-Charnly for the cup implants.Results There was not significant difference in Harris Grade between the two groups. But to the image consideration, the loose extension of acetabular and femular prostheses were severer than the ceramic on ceramic prostheses.Conclusion To young patients, the middle and short time result of ceramic on ceramic is well. But it is so expensive today that the application is limited and the long time result should under clinical observed.
