妊娠期糖尿病患者血清脂肪因子chemerin的表达与糖脂代谢的关系

摘    要:目的:探讨脂肪因子chemerin在妊娠期糖尿病(GDM)患者血清中的表达及其与糖脂代谢的相关性。方法:按照就诊顺序选取2016年12月至2017年4月潍坊市妇幼保健院收治的32例GDM患者设为观察组,选取同期30例糖耐量正常孕妇设为对照组,比较两组血清chemerin、空腹血糖(FBG)、空腹胰岛素(FINS)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)水平,以及胰岛素抵抗指数(HOMA-IR)。分析两组血清chemerin水平与各糖脂代谢指标的相关性。结果:观察组血清chemerin表达水平明显高于对照组,差异有统计学意义(P<0.05);观察组FBG、TG、TC、FINS、LDL-C及HOMAIR水平高于对照组,观察组HDL-C水平明显低于对照组,差异均有统计学意义(P<0.05);观察组血清chemerin水平与TG、TC、HOMA-IR呈正相关(r=0.648、0.532、0.413,P<0.05),与HDL-C呈负相关(r=-0.481,P<0.05),与FBG、FINS、LDL-C无明显相关性(P>0.05)。结论:血清chemerin水平升高与GDM患者胰岛素抵抗密切相关,并且可能参与了脂代谢紊乱的发生。

Abstract:Objective:To investigate relationship between expression of serum adipokine chemerin and glucose and lipid metabolism in patients with gestational diabetes mellitus(GDM).Methods:32 patients with GDM treated admitted to Weifang Maternity and Child Health Hospital from December 2016 to April 2017 were selected as the observation group,while 30 pregnant women with normal glucose tolerance were selected as the control group.The serum chemerin levels,the levels of fasting blood glucose(FBG),fasting insulin(FINS),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),and the insulin resistance index(HOMA-IR)were compared between the two groups.Then,the insulin resistance index(HOMA-IR)was calculated.The correlations between serum chemerin levels and glucose and lipid metabolism index in the two groups.Results:The serum expression of chemerin in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The levels of FBG,TG,TC,FINS,LDL-C,and HOMA-IR in the observation group were significantly higher than those in the control group,the level of HDL-C in the observation group was significantly lower than that in the control group,and the differences were statistically significant(P<0.05).The serum chemerin level in the observation group was positively correlated with TG,TC,and HOMA-IR(r=0.648,0.532,0.413,P<0.05),and negatively correlated with HDL-C(r=-0.481,P<0.05).There were no significant correlations among the serum chemerin level and FBG,FINS and LDL-C(P>0.05).Conclusions:Elevated serum chemerin level is closely related to insulin resistance in the GDM patients,and may be involved in the occurrence of lipid metabolism disorders.
