脑膜病变致脑膜强化的MRI表现及其临床意义

目的　研究脑膜病变致脑膜强化的磁共振成像（ＭＲＩ）表现及其与临床的关系。方法　对４３ 例ＭＲＩ增强扫描表现为脑膜强化的患者进行分析。结果　ＭＲＩ脑膜强化见硬膜型强化３１例，软膜型强化１２例；其中化脓性脑膜炎１４ 例，结核性脑膜炎、脑梗死各７ 例，病毒性脑膜炎、脑膜癌病、血管畸形各５ 例，与临床诊断一致；ＭＲＩ平扫所见正常１２ 例，局部脑膜增厚１１ 例，脑内病灶２０ 例。结论　脑膜强化的类型与病因有一定关系，ＭＲＩ增强扫描有助于发现脑膜病变。

MRI manifestation and its clinical significance in meningeal enhancement induced by meningeal disorders

Objective To study the relationship between postcontrast MR imaging manifestation of meningeal enhancement induced by menigeal disorder and the clinical disorders. Methods 43 cases whose postcontrast MRI showed meningeal enhancement were analysed. Results In postconstrast MR imaging, the patterns of meningeal enhancement were divided into two types: leptomeningeal (12 cases) and pachymeningeal (31 cases). Among 43 cases, purulent meningitis in 14 cases, tubercular meningitis in 7 cases, cerebral infarction in 7 cases, virus meningitis in 5 cases, meningeal carcinomatosis in 5 cases, AVM in 5 cases,they consisted with clinical diagnosis.In precontrast MR imaging , 12 cases were normal, 11 cases were local meninges thicken, others were abnormal in the brain. Conclusions There was certain correlation between the patterns of meningeal enhancement and pathogenesis, MRI enhanced scan contributed to detect meningeal disorders.
