摘    要:目的观察自体髓核组织对神经根的影响。方法成年家兔30只,随机分成实验组和对照组,每组又分为术后1,3,7d组,各5只。实验组在兔L6,7间盘纤维环上钻孔,使髓核组织通过渗漏与邻近神经根相接触;对照组保持纤维环完整。术后1,3,7d,以电生理方法观察各组神经根传导功能的动态变化,同时观察神经根的组织学及超微结构。结果术后1,3,7d,实验组神经根传导速度逐渐下降,分别为(60.12±0.71)m/s、(49.63±1.07)m/s、(30.76±1.36)m/s;神经纤维发生轻微病理改变。术后3d,电镜下可见雪旺细胞肿胀;术后7d,还可见部分神经纤维的施兰切迹增大。结论单纯将纤维环切开,髓核组织通过渗漏可使邻近神经根结构与功能发生改变,造成非机械性神经根损伤。

Abstract:Objective To observe the effect of autollogous nucleus p ulposus on the nerve root. Methods In the experimental group a hole was drilled on f ibrous analus of lumbar 6-7 intervertebral disk, in order that the nucleus pulp o sus was contacted with adjacent nerve root by leaking. In the control group, fib rous anulus was intact. At days 1,3,and 7 postoperatively, dynamic changes of co nduction function of each nerve root, histology and ultrastructure of the nerve root were observed with electron-physiological method. Results In the experimental group, the conducting velocity of the nerve root decreased gradually:(60.12±0.71)m/s, (49.63 ±1.07) m/s, and (30.76±1.36) m/s, respectively, and mild pathologi cal changes could be seen in nerve fibres. At the 3rd day Schwann cell swelling could be observed under electron microscope and at 7th day Schmidt-Lanter man incisure of some nerve fibres enlarged. Conclusions After the fibre anulus is incised, the pulpous nu cleus in it can induce structural and functional changes of nerve root by leakin g, leading to nonmechanical injury to the nerve root.
