前后路一期手术治疗严重下颈椎损伤的疗效分析

目的:探讨前后路一期手术治疗严重下颈椎损伤的临床效果。方法:分析总结采用颈椎前路自锁钢板和后路侧块钢板联合手术复位内固定技术,治疗11例严重下颈椎损伤的临床资料及治疗效果。结果:随访5-24个月,全部植骨均获得牢固融合,颈椎间隙高度及生理曲度恢复满意,脊髓功能按Frankel分级平均有一级以上的改善。结论:对于严重下颈椎损伤实施前后路一期手术是达到充分减压,即刻稳定、为脊髓功能恢复创造有利务件及减少并发症的有效手段。

Surgical Treatment of Severe Lower Cervical Injury by One Stage Anterior and Posterior Approach

Objective: To evaluate the clinical results of one stage anterior and posterior approach in the treatment of severe lower cervical injury. Method: The anterior locking plate systems and posterior plate were used to stabilize the severe lower cervical injury in 11 patients. Their clinical and radiographic records were reviewed retrospectively. Results: The patients were followed up from 5 to 24 months. X-ray film demonstrated complete fusion of autograft and satisfactory recovery of the intervenebral height and cervical lordosis .The function of the spinal cord improved an average of above 1 grade after operation according to the Frankle classification. Conclusion: One stage anterior and posterior approach operation is an effective method in the patients with severe lower cervical injury. This technique may obtain enough decompression and immediate stability, is benefit to the functional recover of the spinal cord and reduce complications.
