肝硬化患者血小板参数变化及临床意义

目的探讨肝硬化患者血小板参数变化及临床意义。方法以198例肝硬化患者作为肝硬化组(分为A、B、C组),同时以100例健康体检者作为对照组,利用全自动血细胞分析仪分别检测血小板参数:血小板计数(PLT)、血小板压积(PCT)、平均血小板体积(MPV)、血小板分布宽度(PDW)、大血小板比率(P-LCR)。结果肝硬化组MPV、PDW、P-LCR明显高于对照组(P<0.05),而PLT、PCT明显低于对照组(P<0.05);肝硬化各组之间比较亦存在显著性差异(P<0.05),随着肝硬化级别的增加,MPV、PDW、P-LCR逐渐升高(P<0.05),而PLT、PCT逐渐降低(P<0.05)。结论血小板参数可作为临床判定肝硬化损害程度及判断预后的重要指标。

Clinical significance of the variation of platelet parameters in patients with liver cirrhosis

Objective To explore the clinical significance of the variation of platelet parameters in patients with liver cirrhosis.Methods One hundred and ninety-eight cases of liver cirrhosis were considered as liver cirrhosis group,which was divided into three groups named A、B、C respectively,one hundred cases of healthy physical examination were considered as control group at the same time.The platelet count（PLT）,plateletcrit（PCT）,mean platelet volume（MPV）,platelet distribution width（PDW） and platelet large cell ratio（P-LCR） were measured by automated blood cell analyzer.Results Compared with control group,the levels of MPV,PDW and P-LCR were significantly higher（P〈0.05）,the levels of PLT and PCT were significantly lower（P〈0.05）.There was statistical significance in platelet parameters in liver cirrhosis group（P〈0.05）,with the aggravation of liver cirrhosis,MPV,PDW and P-LCR were gradually increased（P〈0.05）,and PLT,PCT were gradually decreased（P〈0.05）.Conclusion Platelet parameters can help to determine the loss of liver function and the prognosis in patients with liver cirrhosis.
