新生儿窒息患儿血浆组织因子和组织因子途径抑制物水平变化

摘    要:目的探讨新生儿窒息血浆组织因子(TF)和组织因子途径抑制物(TFPI)含量变化及其意义。方法选择64例新生儿窒息患儿和同期32例正常足月新生儿(对照组)作为研究对象,采用酶联免疫吸附法(ELISA)分别测定脐血、窒息极期和窒息恢复期血浆TF、TFPI含量。结果新生儿窒息时,脐血TF含量明显升高(P<0.05),TFPI含量略有升高,TFPI/TF比值变小,与对照组比较二者差别无统计学意义(P>0.05)。窒息极期,外周血TF含量进一步升高,TFPI含量则出现下降,TFPI/TF比值继续变小,三者与对照组比较差别有统计学意义(P<0.01);重度窒息组TF含量比轻度窒息组高(P>0.05)。窒息恢复期,外周血TF含量明显回落,轻度窒息组与对照组比较差别无统计学意义(P>0.05),但重度窒息组仍然高于对照组(P<0.05);TF含量轻度窒息组比重度窒息组低,但差别无统计学意义(P>0.05);TFPI含量3组之间比较差别无统计学意义(P>0.05);但TFPI/TF比值窒息组同对照组比较仍然明显降低(P<0.05);窒息恢复期TFPI/TF比值轻度窒息组比重度窒息组高,差别无统计学意义(P>0.05)。结论新生儿窒息时,TF和TFPI含量出现明显改变,TF持续升高,而TFPI含量初期升高,极期降低,TFPI/TF比值降低,其变化程度与窒息程度呈正相关关系。
