乌司他丁对慢性心力衰竭患者心功能和炎性介质的影响

目的研究乌司他丁（UTI）对慢性心力衰竭（CHF）患者的心功能以及对炎性介质的影响。方法将入选的60例CHF患者随机分为两组，常规治疗组（n=30）仅接受常规的抗心力衰竭治疗，乌司他丁组（30万U／天，n=30）在常规治疗的基础上加用乌司他丁，观察两组患者治疗前后心哀积分（HFS）、NYHA心功能分级及6min步行实验距离（m）以及血清IL-6、TNF-α、C-反应蛋白（CRP）、IL-10水平的变化。结果治疗后乌司他丁组患者HFS、NYHA心功能分级及6min步行实验距离（m）较常规治疗组均明显改善（P〈0．05）；炎性介质CRP、IL-6、TNF-α、IL-10下降水平较常规治疗组更明显（P〈0．05）。结论在常规治疗心力衰竭的基础上加用乌司他丁，能改善CHF患者的临床症状，对炎性介质起免疫调理作用，减轻炎症反应。

Effect of Ulinastatin on Heart Function and Inflammatory Cytokinases in Chronic Heart Failure Patients

Objective To investigate the therapeutic effect of ulinastatin on inflamma torymediator and cardiac function in patients with chronic heart failure （CHF ）. Methods Sixty patients receiving conventional treatment were randomly allocated to either ulinastatin group （ group B, 1 × 10 unit once a day, n = 30） or conventional treatment group （ group A, n = 30 ）. Changes of serum CRP TNF - ,IL - 6 and IL - 10 levels, conditions of the heart failure score （ HFS ）, New York Heart Association function class, and 6 minute walk test distance were observed in these chronic heart failure patients before and after the seven - day treatment. Results HFS, NYHA function class and 6 minutes walk test distance improved more significantly in group B than in group A （ P 〈 0. 05 ）. Serum CRP, TNF - α and IL - 6 levels reduced more significantly after treatment in group B than in group A （ P 〈 0.05 ）. IL - 10 rose significantly after treatment in group B （ P 〈 0.05 ）, but it didn＇ t change significantly after treatment in group A （ P 〉 0.05 ）. Conclusion Conventional treatment, plus ulinastatin, significantly improves theimmune condition and reduces the inflammatory reaction in chronic heart failure patients.
