HLFO方案治疗晚期胃肠癌

目的:观察羟基喜树碱(HCPT)持续给药联合奥沙利铂、甲酰四氢叶酸/氟尿嘧啶组成的HLFO方案治疗晚期胃肠癌的近期疗效、毒性反应及生存期。方法:21例患者,均为经病理组织学确诊并有远处转移的晚期胃肠癌患者,其中原发病为胃癌9例,大肠癌12例。合并肺转移5例,肝转移12例,骨转移3例,腹壁转移4例。采用含羟基喜树碱(HCPT)5mgciv24h d1~d5;奥沙利铂(OXA)85mg/m2,ivgtt2h,d1;甲酰四氢叶酸(CF)200mg/m2ivgtt2h,d1,d2;氟尿嘧啶(5-FU)400mg/m2,iv,d1,d2;氟尿嘧啶600mg/m2,civ24h,d1,d2,每2周重复。按照WHO标准评价疗效和毒性。结果:全组21例均可评价,获得CR2例,PR8例,SD7例,PD4例,总有效率47·6%,中位生存期(MST)9个月。主要毒性反应为感觉神经毒性、骨髓抑制和胃肠道反应,均较轻微。结论:HLFO方案治疗晚期胃肠癌近期疗效好,不良反应轻,耐受性良好,值得进一步研究。

HLFO regimen in treatment of advanced gastrointestinal carcinoma

Objective:To observe short-term effect, toxic reaction, and survival time of HLFO regimen in treatment of advanced gastrointestinal carcinoma.Methods:21 cases were confirmed by pathology and diagnosed as metastatic carcinoma. Among them, the primary were 9 of gastric, 12 of large intestine. The metastatics were 5 of lung, 12 of liver, 3 of bone, 4 of abdominal wall. The patients were given HCPT 5mg civ 24h d1-5, OXA 85 mg/m~2 ivgtt 2h, d1; CF Acid 200mg/m~2, ivgtt 2h d1-2; 5-Fu 400 mg/m~2, iv, d1-2, 5-Fu 600mg/m~2, civ 24h, d1-2. It was renewed every two weeks. The curative effect and toxicity were appraised according to WHO standard.Results:The total efficiency was 47.6%. 2 case have got CR. 8 PR, 7 SD and 4 PD. MST was 9 months. The main toxic reaction showed slight sensory nerve toxicity, marrow restraint and gastrointestinal reactions.Conclusion:HLFO in treatment of advanced gastrointestinal carcinoma has a good short-term effects, slight adverse reaction and a good tolerance, It is worth studying further.
